My mom is 60 and lives at home with her husband. She gets alcohol from him (he's an alcoholic too) or she makes my sister buy it. As far as insight goes, she has moments of clarity where she realizes and has said that her condition is totally self inflicted but for the most part she isn't aware of the extent of her condition. She tells everyone that she has been sick for a week, in reality shes been having a lot of problems for the last two years and now requires someone to be there 24/7. As far as pain tolerance goes her alcoholic neuropathy kills her but she doesn't have a lot of pain when she falls and hurts her self.
