their's a formal list somewhere on the net that lists them by state not sure
where I found it but that's how i found mine when i went from pediatrics to
adult.

I got offered a great job in montana. But after looking at trying to see where
to transfer my daughters records to there was no place in mt. I would have had
to goto the far east side of North Dakota to get to a clinic. I too also thought
every state had them till I researched it and found out they didn't have one.
Once I found out it was going to be like a 15 hr drive to get to clinic I
decided to stay in indiana. and then of course that was only depending on the
weather.
