Several people have replied to my earlier email, asking for more specific local information or leaflets that they could distribute. I searched long and hard on the Nexus website but couldn’t find anything. Disability North Information Service came to the rescue with a link to this page on the Nexus website:
Accessible travel:

From here I found links to several other helpful pages as well as a leaflet with an application form for a pass that seems to cover most things in a more user friendly format:
Accessible travel (bus):
http://www.nexus.org.uk/wps/wcm/connect/Nexus/Accessible+travel/Bus/
About the Concessionary Travel Scheme:

Application form/leaflet:

Hope this is helpful.
Best wishes,
