Due to internal reorganisation we have a rare opportunity for a speech and
language therapist interested in stroke rehabilitation to join our well established
team on a permanent basis. (This post is partly funded by Langbaurgh PCT.)
You will preferably have three years post graduate experience, with at least
one of these spent treating predominantly adults with acquired communication
disorders. You will also be competent in the management of non complex
dysphagia. You will be able to manage your own caseload and work in a MDT
setting, with senior specialist support available on a regular basis. Car
driving is essential for this post.
You will join an experienced team of therapists delivering acute and community
support to adults in a variety of settings, including the rehabilitation wards
at the
The speech & language therapy service also supports the sub regional
specialties of neurology/neurosurgery and ENT, plus regional spinal injuries,
cochlear implant and cardiothoracic services.

Job Purpose
To
provide a specialist clinical service to clients of Langbaurgh PCT who require
assessment and/or intervention for communication and/or swallowing disorders
following a stroke as part of the Intermediate Care MDT.
To
provide specialist communication and/ or swallowing assessment and intervention
for other in and out patients in the community.

Dimensions
Population
and location served:
Clients
within Langbaurgh PCT who have suffered a stroke and been discharged from the
Acute Stroke Unit at James Cook University Hospital (JCUH). Treatment settings:
the Stroke Rehabilitation Unit at
Outpatient clients at
Major Client groups:
Adults with acquired disorders, especially stroke.
Staffing:
responsible for supporting generalist therapists, the
speech and language therapy assistant and students, including supervision of
students on BSc and MSc courses.
Physical effort:
A combination of sitting, standing and walking. Frequent
need to sit/stand in awkward positions, e.g. when assessing patients at the
bedside. Frequent need to move clients in wheelchairs or assist with
sitting/moving clients in bed. Regular driving between locations.
Emotional effort:
Frequent exposure to distressing or emotional situations
when treating clients with severe communication or swallowing problems or
meeting with their relatives e.g. explaining reasons for and offering support
when clients are unsafe to eat/drink orally, dealing with situations where
clients have suddenly lost much of their ability to communicate and are
unlikely to make significant recovery.
Occasional need to deal with complaints or face verbal
abuse.
Mental effort:
Frequent requirement for intense concentration during
specialist assessment and treatment of communication disorders (including
psycholinguistic assessment of language processing, conversational analysis,
and detailed evaluation of oro-motor function for motor speech and
intelligibility) and assessment of swallowing.
Working Conditions:
Frequent exposure to unpleasant working conditions –
required to deal with sputum and oral secretions on a daily basis in patients
with poor oral hygiene during swallowing assessment.
Frequent exposure to clients with contagious conditions
such as MRSA and c.difficile.
Occasional exposure to spills of urine, faeces and vomit.
Occasional exposure to verbal aggression.
Freedom
To Act:
Guided by general health, service frameworks,
organisational and professional policies.
Responsible for own professional actions.
Manages own caseload and works independently without supervision. Prioritises
caseload on the Guisborough General Hospital Stroke Rehabilitation Unit.
May ask for peer support for very complex cases.

4. Knowledge Skills And Experience
Basic qualification and previous experience:
Degree and licence to practise as a speech and language
therapist as approved by the Royal College of Speech and Language Therapists.
Registered member of the Royal College of Speech and
Language Therapists.
State registration as a qualified speech and language
therapist with the Health Professions Council (HPC).
Two years experience of working with adults, including one
year of specialist experience in acquired disorders, e.g. Stroke.
Specialist knowledge across a range of work procedures and
practices underpinned by theoretical knowledge and practical experience.
Experience of working within a multidisciplinary team within
Stroke
services.
Management experience of a clinical population.
Specialist knowledge and skills training.
Clinical Knowledge and Understanding
Specialist theoretical and practical knowledge of a range
of clinical procedures and practices.
Post-graduate dysphagia training to level one.
Analytical and Judgement Skills:
Makes judgements involving complex facts and situations
that require the analysis, interpretation of a range of options.
Tasks include: Diagnostic assessment skills with
neurologically impaired patients, reporting of complex findings to
professionals and patients / carers, extensive skills in linguistic analysis to
identify level of breakdown and plan management.
Application of theoretical models to meet patients’
requirements.
Identification of own training and development needs via
reflective practice.
Planning and organisational skills:
Plans and organises management of caseload, and individual
therapy programmes.
Includes: management of own time, preparation of resources
and day-to-day administration.
Has responsibility for own caseload management.
Includes prioritisation of tasks, and requirement to adjust plans when rapid
response to referrals is needed.
Assists the senior specialist speech and language therapist
with communication and dysphagia training of nurses and health care assistants
in local stroke multi disciplinary training programme.
Has responsibility for auditing the Speech and Language
therapy service for Langbaurgh PCT stroke rehabilitation unit.
Identification of own training and development needs via
reflective practice.
Physical Skills:
Uses
highly developed physical skills with a high degree of precision, speed and
accuracy. Also provides specialist clinical skills for interpretive assessment techniques and
interventions.
Tasks include: Using precise auditory and perceptual skills
for e.g. using the International Phonetic Alphabet (IPA) to record
patients’ utterances (phonetic analysis), and acute observational skills
for accurate assessment, diagnosis and therapeutic management of clients.
Uses acute visual and auditory awareness of subtle social,
emotional and environmental cues.
Communication Skills:
There is a requirement to communicate highly complex,
sensitive and contentious information, in a verbal or written form, where there
may be significant barriers to acceptance and understanding which need to be
overcome. Communication is often in a highly emotive atmosphere e.g. soon after
diagnosis of stroke.
Skills are required to motivate, negotiate, persuade,
empathise, and to communicate unpleasant news sensitively for e.g. when oral
feeding is no longer safe after a stroke, or when communication is chronically
impaired following a stroke.
Communicates sensitive and complex condition and care related
information to clients, relatives and other professionals in a timely and
appropriate manner e.g. support and explanation to clients when there is a poor
prognosis for communication or swallowing recovery.
Frequently communicates with clients with significant
communication problems where there may be barriers to acceptance and
understanding and where highly developed communication skills of empathy,
motivation and negotiation are required.
Duties and Responsibilities
To manage and monitor own clinical caseload, taking
responsibility for delivering episodes of specialist care.
To provide specialist input to adults with acquired speech
and language therapy disorders, especially stroke. To be responsible for the
assessment, planning and implementation of therapy.
To assess and treat patients with dysphagia following a
stroke. To seek advice from the senior specialist therapist for very complex
dysphagic cases.
To plan and carry out appropriate rehabilitation to meet
individual needs.
To liaise with consultants, medical staff, other
professionals and relatives as appropriate in the acute and community setting,
by attending ward meetings / case conferences.
To assess patient suitability for augmentative communication
aids, and advise on appropriate equipment.
To uphold the values of the Trusts and Local Authority
involved, particularly in relation to dealings with staff and service users.
To ensure compliance with infection control procedures,
recognising risks of infection and taking measures to reduce it.
Responsibilities for Financial and Physical Resources:
To advise the Director of Speech and Language Therapy on
equipment / resource purchases. This may include communication aids, feeding
equipment, computer software packages.
Responsibilities for Information Resources:
To be responsible for own clinical and non-clinical data
entry, text processing and storage of data using paper entry systems.
To keep accurate clinical records of therapeutic intervention.
Responsibilities for Research and Development:
To study current developments and research and to train in
new clinical skills, where appropriate, in order to offer a high standard of
evidence based treatment to patients.
To attend and contribute to relevant special interest
groups (SIGs).
To participate fully in the further development of outcome
measures and clinical audit.
Responsibilities for Education
To complete training courses and achieve competency levels
in areas identified by personal development planning, in order to ensure
maintenance of professional standards in acquired neurological disorders.
To update skills and knowledge on a regular basis by
attendance at specialist course updates, peer review and reading journals.
To provide training to other professionals on both acquired
speech and language therapy disorders and dysphagia within the field of stroke.
To provide mentoring to new members of staff within the
department.
To supervise speech and language therapy students and other
students from varied professions as appropriate / required.
