Thanks for the advice Victoria. If you don't mind I'd like to mention that
technique to our speech therapist. She seems at a loss as to how to deal
with Isaac. He refuses to speak there and it is a waste of our money going
there every week so he can just sit in silence.

Hi Beck,
The first challenge is to have her speak to you in front of others. Find a
reinforcement that he really likes (some kind of sweet and inexpensive items
you can keep on hand). My daughter loves mini-muffins and animal figures.
Other kids like M& M's or whatever. Tell him that he will be rewarded for
"brave talking". Her doctor used "brave talking bucks" .You can use any
kind of computer paper , play money or whatever sort of token to serve the
purpose.
You may need someone outside the family to help. A good psychologist with
experience in SM would be the best. Start by having that person right
outside the door of where you are. Make sure that person is visible to him ,
but far away enough that he can feel comfortable talking to you. Set up a
play situation and STart asking him questions. For each question he
answers, give him a brave buck. After 5 bucks, for 5 questions answered,
he gets a small candy treat. At first he may just whisper the answers.
Pretend you cant hear him. Eventually, the person right outside the room,
needs to move in closer. The point is to desensitize him to speaking in
front of others. A trained child psychologist knows when to move in closer.
Eventually, the other person will be sitting at the table where the two of
you are playing. For a while, you will be asking the questions and he will
be answering you with the other person present at the table. THe psych.
will eventually slip in a question
about the play activity.
At first he may not answer, but if you have a good treat waiting for him, he
might.
This will continue for a while until your son can answer the other person's
questionswith you present in the room. If he has been successful with brave
talking, you might want to give him a special toy (make sure its cheap
enough to keep a few handy). Eventually, you will pretend that you need to
go to the bathroom and leave your son alone with the other person. Now he
will need to only answer the psychologist's questions wiithout you there.
After a while, he will be able to answer questions freely in the office.
Now its time to go out into the community where he will have to answer
questions there. Pick a place that he really likes, candy store, pet store,
dollar store. YOu and the psych. can ask him questions there. At first he
may not be able to, but with enough exposure he eventually will. This will
continue until he is comfortable. In the meantime, you will give him Brave
Talking Goals and keep track of them. The first goal may be to answer
another child's or teacher's questions at school. Try inviting kids to the
house, since he is most comfortable there. Make a big deal when he brave
talks to other kids and teachers.
There is a lot on the subject, so take the first step first. It is a slow
process, (months and months) but it does work. Consider putting him on a
low dose of prozac (10mg.) to lower his anxiety level.
Good Luck,
Victoria

My daughter was always very clingy with me, especially when visiting new
people. As a baby, it was difficult to leave her with anyone who was not a
familiar family member. She never spoke to outsiders, even at her home.
Now she can talk to anyone, although she still remains on the shy side. At
least her reserved personality is now in the normal range. She will start K
in Sept. and I dont anticipate any problems, I hope.

It is strange because before we knew what SM was I'd never really stopped to
consider that Isaac didn't speak in certain situations. I guess because he
always speaks at home. And he talks to close members of our family and
friends that we see often. Now though when I think back I realise that
there are many places that we go where he participates with encouragement,
like Kinder, but never actually speaks. I feel bad that I never really put
it all together before now.

It took me a while to find a good psychologist also. Do lots of research on

Dear Beck,
I am glad you found out about SM. It is a very crippling anxiety disorder.
Please find a competent child psychologist in your area who knows about SM.
Also consider putting him on meds (prozac) to lower his anxiety. My
daughter is making a full recovery from SM thanks to her doctor and meds.
Check out the website called About Our Kids. It has a treatment protocol
for SM by New York University.
Dont let your son continue to be mute for too long, or the mute behavior
will be too entrenched.
SM is curable but it does not go away on its own. Find him help. Good luck

Dear Beck,
I am glad you found out about SM.
Good luck

Ask around and look for someone familiar with childhood anxiety disorders. It
took me a while to find a good psychologist also. Do lots of research on your
own too.

unfortunatly my participation with my daughter at school had a negative effect
it gave her an easy way to not talk by using me to communicate for her the
problem now that she's a teenager is that if I'm around she gets stubborn and
won't try if I'm not around she procrastinates and nothing gets done

Hi
Go to this link:
http://www.imh.com.sg/quietroom/
I belong to the SM Parents' Support Group mentioned in this site. You are
welcome to join us - we meet once a month. If you like more info on what
the group is doing, you can email me privately.
As for your child, you can take him to a private psychologist or
psychiatrist or to cut cost, you could get him to see a doctor at the
polyclinic to get a referral to a government psychiatrist at the Child
Guidance Clinic, Institute of Mental Health.

Wai Kiu

Hi Victoria,
Thank ou for this info. I am very nervous about putting her on medication like
prozac, I always thought it to be some kind of heavy duty drug, but I am willing
to give it a try. She has an appointment set up with one of the psychologists
at Kinark. I'm in Oshawa also.

It is wonderful to start her as young as possible with prozac and behavior
therapy. At first, I tried the behavior therapy without the prozac because I

prozac helped her respond to the therapy and the two together made a huge
difference. She is now talking to kids, teachers, neighbors, store clerks, etc.
SHe is playing and participating in school. Before intervention, she sat by
herself, never spoke, never participated in anything.
The therapy helps to desensitize her to fearful situations, usually speaking in
front of others. Gradually, the therapist brings"brave talking" goals to a
higher level. There is a wonderful treatment protocol on SM on the NYU Child
Study Center website called About Our Kids. Here is where I found her
psychologist.
The only side affect I notice from prozac is that it makes her more active and
talkative, especially at home. I was told there are no long term side affects
and it is not addictive. When she first started, she slept and ate less, but
this went away. I really cant recommend it enough, since we are seeing
wonderful results. I too was scared to medicate her at first. Now I would tell
any parent not to hesitate.
The toughest part is finding a good therapist with experience in SM. Where do
you live?
Good luck,
Victoria

Hi Lena,
That is a good suggestion, one that I will have to try tomorrow when my daughter
goes to daycare. If it's working for your son then you must be doing the right
thing.
Thanks

Hi Victoria,
Thank you for your advise. I am in the process of getting a psychologist
right now, and I will definitly make sure they are experienced with SM. I would
like to ask you, what exactly is behavior therapy and does your daughter have
any negative side effects to the prozac?

Hi, i'm Lena with a 3 yr old son with SM character traits. Just to share my
experience with my son with u. Last week and this week i went to my son's
nursery classes with him. I went there earlier and chatted with the teachers and
his friends in front of him. During the conversation, i got him involved now and
then. This week, i realised he is bolder, he tried to make conversation with his
friends and teachers. Yesterday, he even volunteered to answer questions by the
teacher. i was outside his classroom all the time.
I hope i am doing the right thing.....

Hi, i'm Lena, mother to a soon-to-be 3 yr old boy.
Recently, i had feedback from my son's pre-nursery
teacher that he didn't
join in most of the activities.
i read up about shyness and selective mutism and found
that my son has
characteristics similar to SM.
Where can i find a professional to assess my child's
condition in Singapore?
How do i know that he is having SM????
Anxious mum,
lena

My child is on prozac, 10 mg. It has definetely helped reduce anxiety but it
hasnt taken it completely away. She is still shy in many situations, but can
cope much better now.
Everyone has noticed the difference in her. Good Luck, victoria

I know at least one of you has children with anexity problems other than SM and
i was wondering what if any meds they're using. It looks like SM is only the
tip of the ice burg and my daughter is starting to show alot more anxeity
symptoms. I guess that's why treatment has never worked. Any way any info on
severe anxiety meds would be helpful thanks
