I thought members of the MD-list would be interested to learn that a
Health Care Transition Training Program for youth and families has
been completed and is now available on the Web at
http://hctransitions.ichp.ufl.edu/ddcouncil/
This site has been developed by the Institute for Child Health Policy
with funding from the Florida Developmental Disabilities Council to
help young people with disabilities and chronic health conditions and
their families with the process of planning for and successfully
completing a transition to adult oriented medical services.
This training program is divided into 5 chapters.
Introduction to Health Care Transition
The Consumer Perspective
School, Vocational, and Legal Systems
Promising Practices for Health Care Transition
Developing Health Care Transition Plans
Each Chapter includes 4 or 5 presentations. Each slide show
presentation has an accompanying audio track and is about 15 minutes
long. . In addition, video clips, written documents, and a resource
section round out the educational experience. Users can go through the
training program at their own pace; stopping and starting as they have
time
In Chapter 5 (Developing a Health Care Transition Plan) users learn
about developing a plan that addresses their specific goals and health
care needs. Users also learn how to use Health Care Transition
workbooks. These workbooks help you figure out what you need to know
about transition and help you set goals that support the transition
process.

I know that this issue is a challenging one. We hope that this
on-line training - which provides powerpoint slides and accompanying
audio-presentations will allow youth as well as families to listen and
learn about the issues and ways of addressing the challenges...

a set of health care transition workbooks. These materials are
