Hello Group,

Once a rosacea sufferer has entered "true rosacea", biopsies
indicate that there is always an abnormal neutrophil response in the
rosacea active areas. This is why some derms consider rosacea to be
a neutrophilic dermatose. Neutrophils are not our friends in this
respect.


Over the past two years dozens of rosacea sufferers starting on
singulair treatment have reported significant improvement in their
vascular rosacea symptoms. Why?

Singulair selectively blocks Leukotriene B4 receptor activation –
helping with the neutrophil-induced rosacea inflammation.
Leukotriene B4 is a potent attractant for neutrophils and therefore
may be an important mediator in neutrophil-rich inflammatory
responses.

Why is Zileuton potentially better?

Zileuton blocks an enzyme (not just one receptor) which produces
leukotriene B4 and many dilator leukotriene derivatives.

Through the action of another enzyme called 5-lipo-oxygenase
multiple vasodilator leukotrienes are produced in the skin to cause
blood vessel dilation. This enzyme is activated via many mechanisms
including allergies, heat, irritation, dermatitits, etc. This is
why zileuton may be one of the first true safe anti-inflammatories
for vascular rosacea. The other benefits would be reduced sebum by
65% and reduced papules, pustules and acne by around 75% (studies
entering phase III).
