I had injections into my piriformis muscle with flurescence guidance. First one
was a cortisone and lidocaine injection in June 2004. I wanted botox, but my
pain doctor needed to confirm my piriformis first in his way and he thought
botox was too much risk to begin with. I had some relief but it was short lived.
I think lidocaine helped more than cortisone. I believe cortisone harmed my
metabolism.
I had botox injection in January 2005 (200IU). It was too much for me because it
took me one year to feel strong again. I developed arthritis due to my fault
probably I was not suppose to lift anything heavy and not walk on uneven
surfaces etc. I kept walking on the beach and kept trying to do things(traveling
and carrying suitcases). The problem is when you have botox first you might even
have more pain because your piriformis muscle is totally relaxed and can not
carry anything even itself, but my piriformis was too thick due to overworking
the muscle when it is short. ( I got injured because of yogic flying; I was
jumping sitting in lotus position every morning and evening now it sounds very
silly!) It was painful all the time after tendonitis and when my tendonitis was
healing my nerve was getting more tight. I was in so much pain I told my doctor
I take all the risk just do it. (I read the research papers it should be 100IU
max according to research, I do not recommend more than that mine was too heavy)
I had some trigger point injections(lidocaine) after botox they help with the
pain, but botox felt like my muscle was removed with surgery; therefore got to
behave like you had surgery not to create extra pains like me! The relief comes
after the botoxed (unnerved) muscle developes new nerves. All kind of research
on PS treatment with botox is also on the net. I wrote all this in more detail

After all the weakness and pain my sciatica is not getting tight anymore.
Chiropractor told me that my piriformis is no longer tight, but I've got myself
an arthritic hip. I also have muscle pain around the area when tired, I am
getting better everyday, because these are things that are common to everybody
and there are many remedies, I am taking supplements as I mentioned before. If
you've had a tight sciatica nerve which leaves you so desperate, you will know
what I mean when I say all this is minor pain. I am going to a physiotheraphy
gym three times a week to get strong, I can use all the equipment now and I can
sit as much as I like without being scared!
