There is one thing I would recommend adding to your question list.
"What back up do you have if there is an emergency or complication after
the operation?"

After my second ICL was inserted I had acute closed angle glaucoma. I had
called and called and was repeatedly told it was ok and to go back to
sleep. This was despite severe vomiting that did not subside, extreme pain
and loss of vision in my eye.
Had I not presented to the emergency room there is a chance I could have
gone blind in that eye.

After care is very important and well worth exploring.
