Hi friends, if you have a moment to answer this quick question I'd sure
appreciate it. Not a survey, and not for quoting by name of course, but any
input appreciated,. I have sent this to inquiry to just a few lists on which
I participate.

If you have ever have filled out forms for SS or any other disability
program, or gone through the process, or (I hope) achieved it, what
classification was used medically? Was there a code or a written diagnosis
etc., or anything related as written by doctors or insurance administrators
etc.?
