pain doctors and debt

Hi,
I'm new at this forum, and I'm hoping for some feedback.
 
I have herniated discs in my back and neck. I also have arthritis in my shoulder and am recovering from a BoTox ( we settled out of court for a pittance)  injection that nearly destroyed my right superspinatus ( underneath the trapezius in my left shoulder ) I'm taking the maxium allowable dosage of Hydrocodone, along with the same of Ultram. These don't seem effective to me most of the time, unless I overdose, which I know is dangerous and not recommended by any doctor. I've done epidurals, facette injections and radio frequency lesioning and all helped a little but not for long enough for my doctor to justify repeating them. These three different problems conpsire to make every moment pain filled.
 
I, obviously, struggle with depression and occasionally feel suicidal.I'm sure many of you can relate.I haven't worked for two years and am heavily in debt, and part of this debt is back child support from long
ago. My kids are all in their 20's. I am a recovering alcoholic; sober
since 1989.
 
I have to "validate" my various conditions to creditors and government agencies, but I'm having a hard time getting my PM doctor to provide
what I need. Does anyone have any suggestions on how to get a doctor
to provide verification of my conditions in a manner that will help me
in my struggles with creditors ? I don't expect anyone to lie or exaggerate, but my situation is pretty complex and getting it across
to people is maddening. Obviously, creditors are going to be skeptical
and I can't blame them.
 
I have, thankfully, managed to settle a large child support debt with my ex wife, but the state we used to live is still holding me responsible for [aying money I owe them (AFDC) . I simply don't have it.
 
Any help any of you can lend would be most helpful. I thank you in advance. I'm only going to use my initials for personal reasons.
