Dear collegues:
What I do is look for any of the following:
+ Tonsillar exudate or
+ painful cervical adenopathy or
+ absence of cough
...if is present at least 2 of them besides fever we have 40 to 60 percent of Pharyngitis due to Streptoccus Pyogenes (depends on local prevalence)...then I start ATB (a throat swab culture or a rapid test for strep plus culture previously) and evaluate after 2 to 3 days. But if we have one or less besides the fever the probability is less than 15% and I recomend a rapid test or a control in a few days to evaluate the fever.
The utility of palatine petechia are still not so well defined in the medical literature.
