You have to understand that cancer is not one single entity,
and neither are heart disease, hepatitis, arthritis or any other
disease-complex.
IF I had
to pick one lesson to teach about cancer and its treatment, it is this: cancer
is not one disease. It is perhaps a hundred different diseases.
Because
cancers are so different from each other, cancer patients can be divided into
approximately 100 groups. This calls for a hundred different treatment
approaches.
The treatment that an oncologist recommends to a
patient is individualised to his/her needs and the cancer that he/she has. No
two patients are alike. – AP photo
Take just
one of these 100 groups. Advanced or metastatic breast cancer is common. It is
more common in Malaysia than in the developed world. Patients here often
present to the doctor late either because of ignorance or shame. There are also
patients who originally present with early disease but who refuse chemotherapy
and hormonal therapy after their breast surgery. Their cancer then recurs and
spreads.
Advanced
breast cancer can be divided into four subsets as a first approximation. The
division is based on two molecular characteristics of the tumour: HER2 and ER.
Both HER2 and ER are receptors or docking sites on the surface of the cancer
cell which predict which cancer drug will work. HER2 and ER can be either
positive or negative. Two variables each of which there can be two
possibilities make four groups.
We now
have four groups of patients with advanced breast cancer viz. i) HER2 positive
ER positive, ii) HER2 positive ER negative, iii) HER2 negative ER positive, iv)
HER2 negative ER negative. We approach each of the four groups differently. We
use anti-HER2 treatment for the two HER2 positive groups. We use hormonal
treatment for the two ER positive groups. In addition, we use chemotherapy for
all four groups at some time during their illness.
Actually,
advanced breast cancer (or any cancer) is more complicated than that. Not only
are the four groups I spoke about important, we oncologists also take into
account the tumour burden (how much cancer there is in the body) as well as the
status of the patient’s internal organs. How well are her heart, bone
marrow, liver and kidney functioning? All this will influence the treatment we
prescribe.
Patients
often (or is it always?) compare notes and wonder why their treatments differ
even though they both have the “same” cancer. I hope there is less
need to wonder after reading this article. Some patients demand the oral
treatment their fellow sufferers are on because it is more convenient than
injections.
They want
the chemotherapy that does not cause hair loss. Sure, we can give convenient,
tolerable treatment to them but it may not be the appropriate and effective one
for their cancer.
Some
cancer patients die within a few weeks of diagnosis. Many patients are cured
and survive decades after cancer treatment. They lead meaningful, comfortable
lives. Between the first and second scenario, everything else is possible.
There is much variability even within each subset of cancer patients, let alone
the whole cohort of cancer patients.
Once
again, let me dispel the misconception that cancer is one disease that requires
one therapeutic approach. It is not. It is a 100 different diseases with 100
different treatment approaches. Ozone therapy, Gerson therapy, cytotron
therapy, herbal therapy and qi gong for cancer imply that cancer is one
disease. Even if these alternative and complementary therapies are not meant to
treat the cancer directly but to boost the immune system and palliate, it still
does not make sense.
The
immune system is a very complicated and heterogeneous system that is not so
easily boosted. Palliative medicine has come a long way and is as much on a
scientific footing as oncology. Similarly patients who need palliative care can
be divided into 100 groups which required 100 different approaches.
The more
we know about cancer, the more we find the need to split cancer patients into
more and more groups. It is a generalist’s nightmare and a
reductionist’s dream.
I hope
other medical specialists will cheer me on. Heart disease is not one disease.
Neither is arthritis, diabetes, stroke, hepatitis or tuberculosis.
My
colleagues also dissect the disease complex of their speciality into smaller
entities and treat each one differently.
They are
also faced with patients who tend to “lump” (rather then “split”) and to compare notes. Cultural
beliefs have engendered myths and legends about how the human body works, how
it goes wrong and how good health can be restored.
Moreover
our educational system may not have adequately prepared us to understand
contemporary science-based and evidence-based medicine.
Not only
should science and mathematics be taught in English, perhaps science and
mathematics should also be taught in science and mathematics.

A study has found that oxygen gas can have an
anti-inflammatory effect on back pain
Ozone gas injections are being used to treat back pain, including sciatica.
The gas - a kind of oxygen more commonly associated with the ozone layer
which protects us from the sun's harmful rays - is thought to have an
anti-inflammatory effect and may also boost healing.
A new study found that patients who've had the treatment experienced an
average 25 per cent improvement in symptoms.
Back pain affects three out of four people at some time in their lives.
Although in many cases the pain can pass within weeks, in others the cause can
be more serious.
One of the most common problems is damaged discs. The vertebrae that form
the spine in the back are cushioned by small, spongy discs which. when healthy,
act as shock absorbers for the spine.
However, when a disc becomes damaged - known as a prolapsed or herniated
disc - it may bulge or break open, causing the jelly-like fluid within to flow
out and exert pressure on the nerves.
This leads to sciatica, a pain that may affect the back or even shoot down
the legs.
Conventional treatments include painkillers and anti-inflammatories, but if
the pain is chronic, then surgery is sometimes required to remove part or all
of the damaged disc.
Ozone therapy - a mix of both ozone and oxygen - offers an alternative to
surgery.
It was first used medically more than a century ago, and has been tried for
migraine, ear problems and a variety of conditions.
But the therapy has proved controversial and there are many sceptics who
question its effectiveness.
However, a number of new studies are seeing positive results in its use for
the treatment of back pain.
In a recent trial at the University of Toronto, the gas (combined with
oxygen) was injected into patients aged between 13 to 94 with all types of
herniated discs. All participants showed improved pain and function.
The results were similar to those who had their damaged discs treated by
surgical discectomy, but with the ozone injection the complication rate was
less than 0.1 per cent. The recovery time was significantly shorter, too.
With the therapy, the patient is sedated while radiologists, using imagery,
guide a needle and inject the gas mixture into the damaged discs. The procedure
takes about 20 minutes and patients are up and about shortly afterwards.
Patients were found to have an average 25.7 per cent improvement in back
pain symptoms following the therapy.
Just how ozone could work to beat pain is not clear. One theory is that it
acts on compounds called prostaglandins, which are involved in inflammation and
pain. Another is that the supply of oxygen to the damaged area improves the
blood supply and helps in the healing process.
Although the therapy is popular throughout Europe, here in Britain it is
only available privately or from a few consultants on the NHS.
Philip Conaghan, professor of musculoskeletal medicine at the University of
Leeds and a spokesperson for the Arthritis Research Campaign said that it
welcomed anything that helped intractable back pain, but added that there was a
lack of evidence from randomised controlled trials as to whether ozone therapy
was as effective as other surgical means of treating herniated discs.
He said there was also the possible risk of soft tissue damage at the
injection site, adding: 'We need some carefully selected trials to find out how
effective this treatment is.'

Hi all,
Thanks for letting me be a part of the forum. Someone from the Dr Clark forum
recommended me to check in here.
Well, my grandma (77) came back from the hospital. She started out with
pancratic cancer in early '08, and got it cut out. But then earlier this year
was told she now had liver cancer. Now, just before she left the hospital last
week, they said tests show she might have signs of brain cancer and possibly
lung cancer too (my late grandpa was a chain smoker in the house). Needless to
say, she's depressed. She was sent home by all her doctors to die. She's on
hospice care, now, with family helping out any way we can.
Her mind is just fine, she's conscious and can talk and watch TV and stuff, but
she's very weak and bedridden. Her right arm (her writing arm) is almost useless
but not completely. They said this might be due to a mini-stroke, but are not
really sure of that. She also has had issues with recurrent internal infections
for the past year that keep sending her to the hospital, so her immune system
must be shot. Finally, she suffered some kind of heart failure, which I am not
to clear on as yet. To top it all off, she really doesn't think much of
alternative medicine. She's a product of the 50's and 60's trust in the medical
system. And yet, despite all this, I learned tonight that she was "not ready to
go". My aunt actually heard my grandma say that she was not ready to die.
My uncle and dad are crushed and having a hard time with this news. The main
problem of course is that none of them, including my grandma, is comfortable
with death. However, that's something I have to leave up to each person for
themselves to resolve. In the meantime, I still have hope that she could
recover...if she wants to!
Problem for me right now is that she will never be able to do any kind of a
program that costs thousands of dollars (she's already spending a bundle on care
she receives at home), and she can't do any program that's going require a lot
of work on her part. The Hulda Clark program, for instance, is much too beyond
her or her means, although portions of it could be done, no doubt.
OK, so here's my question for you all. I've heard of low dose Natrexone against
cancer, and she's going to try that starting tomorrow. I'm also going to talk to
her about Mangosteen juice and zapping. But do any of you have other
suggestions that would help boost her immune system that are easy to administer
and affordable? I heard that ozone therapies are a good treatment, but I don't
know that much about it, and honestly, with 2 small children, job, and school
issues to sort out right now, I just don't have a lot of time to do in-depth
time consuming research that should be done. My one beef with the ozone idea is
that it seems like some of those machines are very expensive. At least the ones
people claim are quality machines, anyway.
If it's something that's relatively easy or simple, maybe I can get her to do it
on the basis that "maybe it won't work, but it won't hurt to try". At this
point, she might be more open to alternative methods that sound like they make
some sense in her mind or are easy to do, since her doctors have nothing else to
offer.
