The survey is open to any young person living with HD. No names are required and
the survey is totally OPTIONAL!
There are some tough questions asked on this survey that only you, as a young
person living with HD, have the experience to answer. Most of the questions
address topics that have been discussed at one time or another during NYA
Sensing Sessions at a national convention. The questions were put together by
the NYA Adult Advisors.
Since this survey is 100% anonymous and there is no way to trace back to who
filled out the survey, we are hoping that you will take the time to take this
survey and be as open and honest as you can be. By providing your answers to
these questions you will be helping HDSA Convention Planning committee's in
determining and planning the sessions and programs that can meet the needs of
young people attending the convention. Your response to this survey will also
be shared with HDSA Chapters who have, or will be establishing, HD Youth Support
Groups to help them better understand your needs from a support group.
Again, the survey is optional. You do not have to take it if you don not want
to. However, by taking the survey, you will be helping all young people living
with HD by letting the HDSA and the HDSA Chapters/Support Groups become more
aware of the type of support you need.
If you are a member of an HDSA Chapter or HD Support group, please provide a
copy of this survey the young people in your group. The completed survey should
be printed out and mailed [without a return address] to:
