Here is some info from a very prominent GI researching that I have
contacted regarding:
1) Stay on the 6-MP, particularly if the disease has recently been
active. There isn't a ton of data on 6-MP in pregnancy in IBD
patients, but the studies that are out there all say it's safe. 6-MP
is also used in kidney transplant patients with more potent
immunosuppressants like cyclosporine, and even these people have
healthy babies. What is most clear is that the worst possible thing
for the health of the baby is if the mom has active Crohn's disease
during the preganancy, so any effective therapy is likely a good
thing. About the only medication that we use to treat Crohn's that is
absolutely contraindicated in pregnancy is methotrexate.
It is always important to qualify these types of comments: I do not
know the patient. There are cases where I have stopped 6-MP in
pregnancy based on individual circumstances, and many more where I
have continued it. In other words, there are no hard and fast rules,
and the patient should have a long talk with her doctor about risks
and benefits in pregnancy.
2) OK - that's a tough one (because we really don't know exactly what
6-MP does)! Simply put, 6-MP is related to a chemical that is found in
our DNA, and therefore interferes with a cell's ability to make DNA.
Making DNA is necessary for the expansion of inflammatory immune cells
that create damage in IBD. Therefore, 6-MP stops the inflammatory
cells from growing/expanding. It's certainly a lot more complicated
than that!
