for research participants who have been approved for weight loss surgery by
their physician (though not necessarily approved by insurance!) for my research

Female
AND
Pre Weight Loss Surgery patient, approved for surgery by physician


Complete questionnaires about emotions, eating, and substance use mailed to your
home

Return your responses in the self-addressed stamped envelope provided

Your responses to these questionnaires will be confidential AND your name will
not be written on the forms when you return them

This study is completely voluntary
