this process was quite straightforward for Dave and I also. our policy was with the BNZ and we just contacted them and they sent us the form we needed filled in by our oncologist. The only stipulation was that it was classed as a terminal illness, and GBM is listed as such. I remember at the time we filled in the form we still didn't believe for a second that Dave had a "terminal"ilness...but it was a means to an end at that stage.
It only took a few weeks for the claim to be processed.
