Dear Bob:
I am looking for a skin/wound assessment sheet. I work in a small ICU in
CT and we do not have no bedside sheet for documentation of wound stages,
dressings used/changed, etc. It's driving me nuts!!!!!!! I have looked on
numerous sites-just to get the standard form. I do not know where to get
any--do you? Can you help me?
