One of the things we look for is the A/O of the patient. If the patient is
awake A/O, breathing without difficulty and lung sounds are good, then the
RT will get the weaning parameters. If the weaning paramaters are
satisfactory, then you will usually go to a Bi-Pap/Flow and draw ABGS in 30
minutes to one hour. If the ABGs are good and the patient is still
breathing without difficulty and VSS, then extubation is considered.
Obviously you want to monitor the patient first - is the patient breathing
without difficulty, then your ABGs along with your O2 saturation. Hope this
helps. As always, policies will differ by institution.
