- The Critical Information for Caring for the Parkinson's Patient,
is a tool developed by the Parkinson's Resources of Oregon (PRO) and
the Parkinson Center of Oregon at Oregon Health Sciences University
(OHSU) to assist patients and medical professionals in providing
optimal care for those with Parkinson's disease.
Parkinson's disease symptoms and treatment regimens are often
complex, requiring strict adherence to medication and dietary
schedules. This document will serve as a useful tool to help
communicate this importance to care professionals.
Critical Information for Caring for the Parkinson's Patient provides
an opportunity for a patient to express their unique needs/symptoms
and also serves as an education tool for care staff you interact
with. Because symptoms and medication regimens frequently change, it
is important that a patient periodically review and update this
information.
Should the patient, a family member of caregiver require additional
care and support either at home or in an institutional setting, they
are urged you to ask that the Critical Information for Caring for
the Parkinson's Patient be included with the patient's Care Plan as
well as attached to medical records. Several copies will insure that
all relevant parties have ready access to the information.
