I have been taking four 100,000 heat units for more than a year. My acid reflux was a problem in the past - much less so since I retired (I take no medication for it). The capsaicin seemed to make no difference.
Incidentally, unlike black pepper, which is a stomach irritant, a number of studies have found capsaicin to be gastroprotective.

I am using "Nature's Way". From iHerb.com. They are $5.00 for a bottle
of 100. They also have a 40,000 unit pill at $4.95 per 100. Useful for
getting used to the capsaicin. I did the 40,000 pills for a week or so
before switching to the 100,000 unit pills. Now I can take 6 of the
100,000 pills a day without any problem. I would start low and increase
the dose gradually. It is at least good for arthritic pain, though for
me it seems to be good for the PC bone pain. At least as good at
Opiates. A year's supply costs less than any one test or medication
done by the Hospital.
To guess a dose, I read the paper and assumed that the "Scoville" units
mentioned in the paper were the same as the "heat" units on the bottle
label. I think I estimated that 3-4 of the 100,000 unit pills a day
would match the dose in the paper. But it is just a guess. I am very
careful to eat food before *and* after taking the pills. A pill
sandwich in my stomach. At first there was some rectal burning, but
this has worn off. I am curious if anyone else has similar results to
mine. I do tell my doctor, and since my PSA has stabilized now below
the peak value he says "keep doing what you are doing". A good guy. He
does not seem insulted that I am trying things on my own.

How did you determine dosage of 200,000-4000 units per day?
