Kaiser Regional/East Bay is seeking a social work case manager to partner with a nurse and provide intensive, home-based case management services to patients with chronic and complex medical and mental health conditions. Since this is a Medi-Cal only positions, these patients are likely to be younger than 65 and to be public sector patients who converted their Medi-Cal over to Kaiser. This is a full-time 2-year pilot position, requiring an innovative spirit and someone who wants to work hard to develop and evaluate the program further. This is perfect for an MSW graduate with health care experience.
