Position Title: Case Manager (Bilingual in English and Chinese)
Salary/Benefits:
$ 32,000 – $ 36,000 FTE annually with

Asian Perinatal Advocates (APA) is a community based organization focusing on the prevention and identification of child abuse/neglect in the Asian and Pacific Islander community.
Our mission is to promote healthy families and prevent child abuse and domestic violence in the Asian/Pacific Islander communities. APA provides family support, case management, and parent/community education services on issues of domestic violence, child abuse, and parenting for high risk families with children ages 0-14. We currently have an opening for the Case Manager position.
Major Duties and Responsibilities:
1. Conduct needs assessment for families referred to APA.
2. Develop and implement case management plans.
3. Coordinate case plan with other staff and services being provided to the client.
4.
Provide direct services and support services, including regular in home visitation.
5. Maintain and compile client records and service documentation.
6. Provide advocacy assistance pursuing related issues.
7. Conduct groups and activities and facilitate interaction among clients
and involvement in the participating programs.
8.
Assist in providing emergency client related services.
9. Perform post-service evaluations.
10. Prepare program and outreach materials.
11. Participate in regular staff meetings, staff development training and other meetings.
Minimum Qualification:
- Bachelor Degree in social work, psychology or other human services field; or a combination of relevant education and experience.
- Experience in providing direct services with families and children.
- Knowledge of child development, parenting skills and API community resources.
- Effective case management, advocacy, organizational and communication skills.
- Written and oral fluency in English and Cantonese. Fluency in Vietnamese a plus.
- Ability to work as a team member and work collaboratively with other community based organizations.
