Bereavement Coordinator to you:

DEPARTMENT: Hospice
SCHEDULE: Part-time Regular; 32 hours/week;
Variable shift; Sunday - Saturday.
EDUCATION/LICENSE/CERTIFICATION: Masters
degree in Counseling, Social Work or related field. Current professional
licensure (LCSW or MFCC), or certification.
Current CPR preferred, CA Driver's
License preferred, LCSW in state of California preferred.

POSITION SUMMARY: Provides fiscal management,
designs and provides training, and monitors the on-going bereavement activities.
Designs, implements, evaluates
and improves the Hospice bereavement program. Ensures that high quality,
coordinated bereavement services are provided to patients, families and/or
caregivers. Provides bereavement services that add value and are
integrated with strategic/organizational goals and objectives.
QUALIFICATIONS: Previous experience
in program management, supervision, or oversight responsibilities, as well
as direct counseling work with individuals/groups, in a health care setting
(usually 3 years). Previous counseling experience (usually 3 years). Experience
in budget management, program planning, program development, and strategic
planning experience. Understanding of new and innovative organizational
change processes. Demonstrated expertise in working with grief /loss issues
and crisis intervention. Must be able to work in a Labor/Management Partnership
environment.
PREFERRED QUALIFICATIONS:
· Required
Minimum Experience (typically): Previous bereavement administrative,
supervisory, and counseling experience, preferred. Works independently
without direct supervision; is highly motivated and responsible; has strong
analytical thinking skills and objective problem-solving skills; good judgment.
· Required
Age Specific Competencies: Appropriate age specific competencies to
be successfully completed by end of orientation: Infant, Toddler, Child,
Adolescent, Adult and Geriatric.
· Physical

· Language
Skills: Reading, writing and verbal communication.
· Mathematical
Skills: Knowledge of arithmetic concepts and ability to use fraction,
percentages, ratios, addition, subtraction and division. Ability
to use dates and understands principles of budget monitoring and statistical
analysis.
Computer Skills: Knowledge
of Windows operating system and MS Office programs; ability to learn new
software, (e.g. 3M CDS).

DUTIES: Consults and provides resource
to designated interdisciplinary team members to maximize patient care outcomes
and resource utilization. Ensures compliance with federal, state, and local
regulations. Provides supervisory oversight regarding skills development,
individual competency, capability and positive outcome participation as
a team member in the development of integrated care plans, focusing on
collaboration, and results oriented implementation functions. Manages staff
to meet administrative expectations in the areas of standards of practice,
policies and procedures, annual departmental goals and objectives, quality
improvement, and licensure and accreditation. Designs, implements, monitors
and evaluates the bereavement program services on an on-going basis for
effectiveness and applicability. Oversees the bereavement volunteers. Responsible
for developing and maintaining community relations and all community liaison
activity. Provides primary lead for outside presentations, convening informational
workshops, meetings and training sessions. Participates in the departmental
quality assurance program, evaluating the social services and bereavement
activities. Monitors and maintains the budget. Identifies and recommends
opportunities for cost savings.

Provision of Care
1. Develops,
plans, and directs the bereavement activities for survivors.
2. Provide
services to the family to assist them with adapting to the death of the
family member.
3. Assures
that bereavement services shall be available to caregivers before and after
the patient's death and available to the family for a period of at least
one year following the death of the patient.
4. Coordinates
and provides on-going education and orientation programs for all staff
and volunteers providing bereavement services in compliance with licensing
and accreditation requirements.
5. Develops
processes to evaluate and continuously improve bereavement program.
6. Identifies
new opportunities both internally and externally for bereavement service
improvement.
7. Ensures
volunteers and staff providing bereavement services are in compliance with
federal, state and local requirements.
Documentation and Communication
1. Ensures
bereavement program compliance with licensing and regulatory requirements.
2. Provides
ongoing information to agency administrator on bereavement services.
3. Documents
the assessment and services provided in the Hospice clinical record.
Planning, Organizing and Productivity
1. Plans and
organizes work assignments, set priorities and completes work with a minimum
of supervision per agency policy.
2. Adheres
to the service policy and principles of the Kaiser Permanente Medical Care
Program.

in a courteous professional manner with patients, health professionals
and maintains patient confidentiality.
2. Participates
in Departmental Quality Assurance activities and Utilization Review activities
as requested.
