I feel very violated when my physician filed the claim with medical
insurance company for an office visit I did NOT schedule. I did not
make that appointment. The insurance company issued a check (i.e.
deduct $ from my HRA account) to this Service Provider for this
claim.

The insurance company told me that it's the law to pay for any claim
filed by Service Provider even though I told them that I never
scheduled that appointment. "This is the issue between you and this
provider." was the response from the insurance company.

When I confronted with the physician, I was amazed how justified he
was. According to him, this office visit claim was a re-file for
the claims that were not paid earlier this year. "it's not
fraudulent billing at all" he emphasized in his letter.

To me, his reasoning does not make any sense to me at all. I was
with Different insurance company before July this year, I paid all
my co-payments during all of my office visits. If he had an issues
with that insurance company, that's between him and that company.
How could he file a new claim to my NEW insurance company and felt
justified about that?

It's all about money, never about patient care. I refuse to be a
victim and I will explore my options to get my money back.
