Federally recognized Native American Tribe or Tribal Organization that will deliver services on a Reservation or federally recognized Tribal lands. The applicant organization must represent three or more separately owned entities. Existing networks that seek to expand services or expand their service area are not eligible to apply for this program. |

The grant program supports one year of planning to develop and operationalize health care networks, consisting of at least three health care providers in rural areas. Identify potential collaborating network partners in the community/region. Convene potential collaborating network partners, conduct planning activities, which could include undertaking a community assessment identifying the most critical need of potential network partners to ensure their viability. |
