Exploring the relationship between preferred and actual mode of delivery in women with a prior cesarean birth.
MedWire News: Most women who have had a prior cesarean plan a vaginal delivery at their next birth, but they usually do not get it, a study of UK women suggests.
The finding, which contrasts with the "common opinion" that women opt for an elective repeat cesarean section (ERCS) to avoid labor, highlights the need for strategies to achieve planned vaginal delivery, the researchers say.
Dan Selo-Ojeme (Chase Farm Hospital, London) and colleagues studied the medical records of 215 women with a history of one cesarean delivery.
Overall, 55.3 percent of women chose to have a vaginal birth after cesarean (VBAC), of whom 37.8 percent received it. In contrast, 94.8 percent of women who opted for an ERCS had that mode of delivery.
Afro-Caribbean and Asian women were 3.5 times more likely than white women to choose VBAC, and three times less likely to deliver by that mode.
Factors other than race also influenced the women's preferred mode of delivery. Women who chose VBAC were twice as likely as those who chose ERCS to be multiparous and 90% less likely to smoke, the team reports.
Selo-Ojeme and team concludes that efforts to reduce the increase in cesarean deliveries must recognize the contribution of women with a previous cesarean delivery and develop strategies to achieve a planned vaginal birth.
"Only then can a realistic result be expected," they say.
