Infectious disease surveillance for the London 2012 Olympic and Paralympic Games

The London 2012 Olympic and Paralympic Games will be one of the largest mass gathering events in British history. In order to minimise potential infectious disease threats related to the event, the Health Protection Agency (HPA) has set up a suite of robust and multi-source surveillance systems. These include enhancements of already established systems (notification of infectious diseases, local and regional reporting, laboratory surveillance, mortality surveillance, international surveillance, and syndromic surveillance in primary care), as well as new systems created for the Games (syndromic surveillance in emergency departments and out-of-hours/unscheduled care, undiagnosed serious infectious illness surveillance). Enhanced existing and newly established surveillance systems will continue after the Games or will be ready for future reactivation should the need arise. In addition to the direct improvements to surveillance, the strengthening of relationships with national and international stakeholders will constitute a major post-Games legacy for the HPA.

Directorate General of Employment & Training (Ministry of Labour, Govt. of India) has released the National Occupational Classification of India. It is high time that public health professionals start using this classification in their researches to ensure comparability between different studies in India. Arbitrary classifications would not have any credibility.

There is a provision of fee in RTI and I could get a pdf of a circular regarding this:

"Thus, there is provision for charging of fee only under Section 6(1) which is
the application fee; Section 7(1) which is the fee charged for photocopying
etc. and Section 7 (5) which is for getting information in printed or electronic
format. But there is no provision for any further fee and if any further fee is
being charged by the Public Authorities in addition to what is already
prescribed under Section 6(1), 7(1) and 7(5) of the Act, the same would be in
contravention of the Right to lnformation Act. The "further fee" mentioned in
Section 7(3) only refers to the procedure in availing of the further fee already
prescribed under 7(5) of the RTI Act, which is "further" in terms of the basic
fee of Rs.10/- . Section 7(3), therefore, provides for procedure for realizing
the fees so prescribed".

Also this act provides access of information to Indian citizens only.
An Act to provide for setting out the practical regime of right to information for citizens to secure access to information under the control of public authorities, in order to promote transparency and accountability in the working of every public authority, the constitution of a Central Information Commission and State Information Commissions and for matters connected therewith or incidental thereto.

I think that when these provisions are there in the act, they can ask for these documents.

Thank you very much. But I am also reproducing the information from one of the friends from PGI and recent decision by CIC on citizenship issue as below:

"Even PGI RTI cell included the clause of providing citizenship proof which later on was removed. You can see the present application form of PGI here 

One need not to use a form for seeking information under RTI Act. one can even write application on a plain paper.

In A N Prasad v/s Indian Army (CIC/SM/C/2009/000405/LS) CIC observed "we are of the opinion that the proof of citizenship is not required from an information seeker as a matter of principle.  However, in certain exceptional circumstances, where the CPIOs, particularly of the Armed Forces, have a doubt about the citizenship of the information seeker, it is open to such CPIOs to seek proof of citizenship.  The Commission directs that the CPIOs would exercise this option only in exceptional cases."

There is a provision of fee in RTI and I could get a pdf of a circular regarding this:

"Thus, there is provision for charging of fee only under Section 6(1) which is
the application fee; Section 7(1) which is the fee charged for photocopying
etc. and Section 7 (5) which is for getting information in printed or electronic
format. But there is no provision for any further fee and if any further fee is
being charged by the Public Authorities in addition to what is already
prescribed under Section 6(1), 7(1) and 7(5) of the Act, the same would be in
contravention of the Right to lnformation Act. The "further fee" mentioned in
Section 7(3) only refers to the procedure in availing of the further fee already
prescribed under 7(5) of the RTI Act, which is "further" in terms of the basic
fee of Rs.10/- . Section 7(3), therefore, provides for procedure for realizing
the fees so prescribed".

Also this act provides access of information to Indian citizens only.
An Act to provide for setting out the practical regime of right to information for citizens to secure access to information under the control of public authorities, in order to promote transparency and accountability in the working of every public authority, the constitution of a Central Information Commission and State Information Commissions and for matters connected therewith or incidental thereto.

I think that when these provisions are there in the act, they can ask for these documents.

"Even PGI RTI cell included the clause of providing citizenship proof which later on was removed. You can see the present application form of PGI here 

One need not to use a form for seeking information under RTI Act. one can even write application on a plain paper.

In A N Prasad v/s Indian Army (CIC/SM/C/2009/000405/LS) CIC observed "we are of the opinion that the proof of citizenship is not required from an information seeker as a matter of principle.  However, in certain exceptional circumstances, where the CPIOs, particularly of the Armed Forces, have a doubt about the citizenship of the information seeker, it is open to such CPIOs to seek proof of citizenship.  The Commission directs that the CPIOs would exercise this option only in exceptional cases."

He points at the weak links in the on-going poverty alleviation, employment guarantee and food security programmes. He argues for regular and effective monitoring and reporting systems to help identify real gaps on the ground, work towards better performance and delivery of services.
 
 
Dear Members,
 
I would like to share my response on 'Improving India’s Performance in Millennium Development Goals. 
 
India’s excellent economic growth has had little impact on its social indicators, and India is likely to miss achieving the Millennium Development Goals in respect of hunger, poverty reduction, health, nutrition, sanitation and gender. Two recent reports confirm this. According to the latest Global Hunger Index Report, India continues to be in the category of those nations where hunger is ‘alarming’. What is worse, India is one of the three unfortunate countries where the hunger index between 1996 and 2011 has gone up from 22.9 to 23.7, while 78 out of the 81 developing countries studied, including Pakistan, Nepal, Bangladesh, Vietnam, Kenya, Nigeria, Myanmar, Uganda, Zimbabwe and Malawi, have all succeeded in reducing hunger. Second, the recent HunGama Survey Report released by the Prime Minister in October 2011 shows that there has been no decline in the number of malnourished children since 1998 which continues to be above 40 percent. This dismal performance is despite the central allocation for programmes in the social sector, including agriculture, often going up by almost 10 times in the last decade. Unfortunately many central programmes have a serious design flaw, besides being tarred by inappropriate policies and total lack of oversight.
 
·         The government should re-examine the focus on self-employment in poverty alleviation programmes such as Swarjayanti Gram Swarozgar Yojna (SGSY) and Swarna Jayanti Shahari Rozgar Yojna (SJSRY). The capacity of the poorest to absorb credit and to start enterprises is limited because of their lack of business skills, illiteracy, their inability to take risks and their lack of motivation for business. It would be much better to upgrade their skills and make them employable in the newly emerging industries and trades. The poor would prefer to be wage employed on a regular basis at a decent salary due to improved skills, rather than face the risk of operating in an uncertain market plus the ignominies of humiliation from the insensitive police and municipal officials. Hence the National Rural Livelihood Mission should focus more on skill up-gradation than on self-employment.
·         The stated objective of Mahatma Gandhi National Rural Employment Guarantee Act (NREGA) is to do drought proofing and increase agricultural production on marginal holdings, especially in semi-arid regions and uplands, but the sustainability and productivity of assets created is never monitored with the result that the programme is reduced to creating short term employment (often on paper) with no focus on asset creation or soil and water conservation. Its impact on agriculture may even be negative, as alleged by the Ministry of Agriculture. In any case it makes no sense to run the programme in labour scarce districts of Kerala, Punjab, Himachal, and the north-east. On the other hand, the upper limit of work guarantee of 100 days should be enhanced to 150 days per household in the poorest 200 districts, with assets such as ponds, bunds, check dams, and saplings being monitored at least for five years.
·         Major food related programmes, such as the Public Distribution System (PDS) and Integrated Child Development Services (ICDS) are plagued by corruption, leakages, errors in selection, and little accountability. They also tend to discriminate against and exclude those who need them most; compounded by state hostility to poor urban migrants, street and slum residents, dispersed hamlets, and unorganised workers such as hawkers. In Rangpur Pahadi, a slum area just a few kilometres away from Vasant Kunj in Delhi, people living since 1980 have not been given a voter ID card or a ration card. Thus their very existence is denied by the Delhi government!
·         A recent evaluation of ICDS in Gorakhpur by the National Human Rights Commission showed that despite Supreme Court orders to provide hot cooked meals, all centres supplied only packaged ready-to-eat food, which had only 100 calories, as against a norm of 300 calories, and 63 per cent of food and funds were misappropriated. The food being unpalatable, half of it ends up as cattle feed. However, such reports, though few, are never discussed in state Assemblies, as they meet now for less than 30 days a year. We need a new law making it compulsory for Parliament and Assemblies to meet for at least 150 days a year.
·         Moreover, state governments actively encourage reporting of inflated figures from the districts, which renders monitoring ineffective and accountability meaningless. Objective evaluations show that 43.5 per cent children are underweight, of which 17 per cent are severely malnourished. However, the state governments report 13 per cent children as underweight, and only 0.4 per cent as severely malnourished (India Human Development Report, 2011). One district Collector, when confronted with this kind of bogus figures, told me that reporting correct data is “a high-risk and low-reward activity”! Prime Minister may call our performance as a 'national shame', but he has not been able to persuade the states to accept that the problem exists! Forget about the solutions.
·         We certainly need more government teachers and doctors but what if almost half of them are absent, and busy in private practice? It is deplorable that neither the GOI nor the states monitor and measure absenteeism regularly. Policy makers admit that huge errors of inclusion and exclusion exist in identifying the poor but when did we last measure this? Seven years back, in 2004-05. When was PDS last evaluated? Nine years back, in 2002-03. Surely, more frequent assessments by GOI could have put pressure on the states to improve delivery.
 
Incidentally, the recent joint statement of the Deputy Chairman, Planning Commission and the Minister of Rural Development that the present state-wise poverty estimates will not be used to impose any ceilings on the number of households to be included in different government programmes, will further encourage them to inflate the number of the poor in their states. In other words, so far with a cap on each state when a non-poor was selected as a BPL household it was at the cost of a poor in the same state, and it did not increase the total number of poor beneficiaries in that state because of the state cap. Once the cap is removed, showing more deprivations in a state will be advantageous to that state as it will be at the cost of the poor of other states. We will thus incentivize and encourage wrong reporting. Inter-state bickering will also increase as the states with less number of priority households will blame other states of deliberate fudging their numbers. 
 
To sum up, whereas the problems of governance deficit in delivery are well known, the track record of GOI Ministries in formulating and monitoring policies and programmes which can alleviate poverty cannot be called satisfactory. The gap is most glaring where the poorest of them all, tribals are concerned. Persistent problems faced by tribals—land alienation, indebtedness, government monopoly over minor forest produce and the non-implementation of the Forest Rights Act, 2006, among others—have by and large remained unattended. The current approach of the Ministry of Tribal Affairs (MoTA) is to confine its attention to its own budget and the schemes that are under its control, such as grants to NGOs, scholarships, etc. MoTA does not put any pressure on other ministries that have been vested with the responsibility to ensure that basic justice and development reaches the tribals. MoTA does not even monitor whether the basic services in education, health or nutrition are reaching the tribal hamlets.
 
PMO will do well by looking at the performance of its own Ministries rather than only blame the states for poor delivery.

The Wada Na Todo Abhiyan, a campaign set up by a coalition of Civil Society Organisations for monitoring the implementation of the Millennium Development Goals in India is undertaking a consultative process for future Agenda (post-2015) with regard to the MDGs. This process has been initiated by the United Nations in recognition of the fact that all MDGs will not be met by 2015; and global scenario has changed considerably since 2000 so a new global development framework needs to be developed that looks at contemporary economic, social and environmental challenges. In continuation of my work towards Safer Injections, I need your opinions and quotes from literature if any on the following question.

Background: Managing the sharps waste remains a challenge. After disinfection at the point of generation, this is expected to be shredded and then disposed of in a landfill or sharps pit. However, most of the facilities provide electric needle burners.

Question: Is needle burning a recommended best practice for sharps management? 

Our view: The metallic needle gets aerosolized due to electric current. The hollow of the needle contain blood or body fluid which is potentially infectious and gets vaporized in the hospital corner. The health care worker over a period of time gets cumulative dose of such fumes and has the potential risk of contracting infections. Besides, there is blood splatter all around the needle burners. 

Please do not hesitate to share your views and experiences. If any of students has undertaken any primary or secondary research, pl do let us know that as well.

It is state govt. responsibility to close all illegal pathology laboratory in state.

I am sending also Andhrapradesh govt affidavit in favor of pathologist. Only qualified and registered MD patholigist can run pathology laboratory in A.P.

I am sending IMC letter that DMLT technician can't practice pathology laboratory independently.

Dear All,

All of us aspire to be good managers cum good leaders. Hope I am right in saying so. How many of you know the value add that you have brought to the teams/institutions in which you are working. Management teaches us to define our individual goals for one year, undertake mid term evaluation and also assess ourselves at the end of one year. This exercise needs to be repeated every year without fail. 

As I understand, most of you do not have a formal evaluation system. 

If it is not possible to introduce a formal evaluation system in your work place, still please do not hesitate to define your Individual Impact Goals (IIGs) or Key Result Areas (KRAs) for a year and measure yourself against them, irrespective of your seniority or stature. It helps you to develop yourself better, aim for higher goals or attain more skills like communication, knowledge, clinical or managerial skills if you maintain an appetite to achieve better. You should be able to answer a simple question--What Have I contributed to the Community (Medicine) or Public ( Health) in the last one year and Is it worth my experience, knowledge and position?

Additionally, I have always been saying through this forum--Take a look at all the research works undertaken in your institution and answer the simple questions---How many were original research works? The results of how many been leveraged for some meaningful action--howsoever small the action may be?

Intermediate hyperglycemia, also known as pre-diabetes is a condition where the body has a higher concentration of blood glucose than normal, but lower than the established thresholds of diabetes itself.
