Hi all
First, thank you for an enchanted evening yesterday. I was so happy to meet with you all, and had a great time (worth every minute of my hour drive in each direction...).
 
I am forwarding an email I got from a woman who's looking for a doula (EDD 6/23, Yale New Haven Hospital, CT). I am now working with a client too close to that date, and my fellow CT doulas are busy too. Can anybody help?

Hi Sivan, I am looking for a birth doula for this
birth, I did not have a doula for my first, (wish I
did in hindsight!!) I had a relatively successful
hypnobirth 2 yrs. ago, but a 3 hour pushing stage
where the midwife was too loud, too barking at me to
work harder!, work harder!, etc.  which left me quite
UNrelaxed, and discouraged.  I am hoping for doula
support this time to assist me in keeping up with my
relaxation during that stage.  Rachel Bee recommended
you highly. I know it is late to be looking/asking -
so please contact me as soon as possible and let me
know if you are available, and then we can meet and/or

Janet,
Thanks for offering to pick up the samples, but someone else asked if they could have them for a baby shower they are doing for pregnant moms who are using WIC.  They will be picking them up.  

But another thing I wanted to know, is if you are having any childbirth class series starting soon.  I met with a couple last night, and gave them your name.  She's a med student though with a crazy schedule, so I'm not sure if a series will work for them.  I encouraged them to try and find one that would work with their schedule because they would get so much more out of it than a weekend class.  She's due mid-August, and her name is Kathi.   If you could email me your dates, I will pass them on to her.   Thanks!

I live in White Plains and would be happy to pick up the samples. I could do it almost any time (except between (9:30 and 12:30 tomorrow).  Saturday and Sunday I'm busy all day but other than that I can do it. I won't be able to come to the next meeting as I am teaching on Tuesday evenings...but I could probably get them to Katherine and she could bring them for distribution.
 
Just let me know, I'd be happy to pick them up.

If anyone is near White Plains sometime this week, and would like to pick up about 60 sample bottles of Whole Body Baby soap from the Whole Foods Market, please let me know.   She was trying to get them ready to have for our workshop, but they got misplaced at the store when I went to pick them up on Sunday.  She now has them there and wanted to know if anyone wanted to take them for clients or a clinic somewhere.  If you'd like to pick them up, let me know when you think you can get over there so I can let my contact know to be expecting you, as well as pass on her info to you.  Thanks.  I hope someone can get to them and find people who could use them!

Congrats Melissa on a great conference. You (and all your key helpers) did a fantastic job. The day was beautiful, the setting was beautiful and Penny is a great speaker. I really enjoyed it.

I feel very very fortunate that this group reestablished itself as I was starting my certification as a birth doula and childbirth educator. I honestly know I wouldn't be almost to completion if it wasn't for this group and the inspiring women in it.

Hi,
Can anyone just perhaps mail me off list about these two facilities? I
have a mom unsure whether to change from one to another to follow her
OB or to stay put and change OB.
I don't have any experience of either, so would like to just hear if
anyone thinks one has a lower intervention or c-section rate than the
other or one is particularly good......whatever that means!! I
apologise that the question is somewhat vague, I just thought I might
be able to dig around some other people's experiences and thoughts.

I'm looking for some information to share with a
friend/client about VBAC (placental abruption first
time round. Cesarean just barely saved her and  baby's
life). She is eager for a VBAC this time 'round.
She just interviewed an OB in the city (Dr. Juanita
Jenyons) whom I had recommended as a VBAC-friendly ob.
The interview pretty much started with Dr Jenyons
saying that new studies have shown that VBAC babies
have a slightly higher of incidence of asphixia at
birth than those born by elective cesarean. She then
went on to recommend a second cesarean to my friend
since she does not plan on having more babies. I have
since learned from others that while supportive of
women who choose VBAC, Dr. Jenyons is generally
reversing her stance on VBAC.
I am wondering how to discuss risk with my
friend....How to help her to find information that is
pregnancy-compatible. You know what I mean?  She is,
after all, a pregnant woman wanting to enter birth
with a  stong faith in her body and the process of
birth. She does not want to be reading as a health
practioner would about risks of maternal death, fetal
asphixiation etc. And yet, she needs to make an
informed decision here. She needs to weigh risks and
benefit and feel at peace with her decision.  I'm
wondering if anyone has advice about ways to
think/read/reseach about risks associate with VBAC and
elective C/s  when pregnant???

Rarely this is associated with a fetal anomaly. I assume the
ultrasounds have shown the baby to be fine or you would have
mentioned the anomaly. It shouldn't be a problem and they should
leave her alone. Susanrachel

I just had a birth client sign up for my doula services, and she has been told that she only has 2 blood vessels in the umbilical cord, instead of the normal 3.  For those of you who have dealt with this, can you please tell me if this would interfere with the mom wanting to labor naturally?  Would she have to be monitored more?  I realize it depends on the OB and hospital, but any insight would be helpful.  Thanks!

Fat Content of Breast Milk Increases with Time

TUESDAY, Sept. 6 (HealthDay News) -- The longer a mother breast-feeds, the higher the fat and energy content of her breast milk .
However, experts are not sure what this finding, which appears in the September issue of Pediatrics, signifies.
"This is the first study to analyze the fat and energy content of breast milk of mothers who breast-feed for longer than a year," said study co-author Dr. Ronit Lubetzky, who is with the department of pediatrics at Dana Children's Hospital at Tel Aviv Sourasky Medical Center in Israel. "There are more and more women who choose to breast-feed for longer time periods, and not many studies about the nutritional value of their milk during this prolonged lactation."
"This is a nicely done study which looked at a question that really needed to be answered," added Dr. Ruth Lawrence, a professor of pediatrics at the University of Rochester School of Medicine and a member of the executive committee of the American Academy of Pediatrics' section on breast-feeding. "I think many people's general impression is if you continue to breast-feed beyond a year, probably the nutrient value drops, and this is quite different information and very important."
No one is sure how long mothers should breast-feed, although the American Academy of Pediatrics recommends that "breast-feeding continue for at least 12 months, and thereafter for as long as mutually desired."
A reduction in cardiovascular risks in adulthood is one oft-cited benefit of this practice. Others, however, have said it might have the opposite effect.
To determine the fat and energy content of human breast milk at longer periods, Lubetzky and colleagues sampled the breast milk of 34 mothers who had been breast-feeding for 12 to 39 months, and compared that with the milk of 27 mothers who had been breast-feeding for only two to six months.
They found a startling difference: the fat content in the mothers who had breast-fed for longer periods of time was 17.5 percent, versus only 5 percent in the short-term group.
The researchers said that, while it was possible that something other than duration might be affecting the findings, they still felt this was the most likely explanation for the difference.
It's not clear what the effects of this higher energy and fat content are on a child's health.
"We showed that the milk of mothers who breast-fed more than a year had a very high fat content," Lubetzky said. "That contradicts the claim that breast-feeding at this stage has no nutritional contribution. On the other hand, the long-term effect of such a high-fat intake has not been studied."
"The constituents of fat and human milk are very different than what we provide in formula today. One of the most important constituents of human milk is cholesterol. Formula does not," Lawrence said. "There are many people who think that probably one of the problems with cholesterol today occurs because infants have not had any cholesterol in the first few months of life; perhaps the body doesn't learn to deal with it. There are studies that show that young adults have much lower cholesterol levels if they were breast-fed than if they were bottle-fed."
Still, Lawrence added, this is an area that needs to be researched further.
Lubetzky agreed. "Further studies should analyze this milk fat qualitatively, and try to sort out the influence of prolonged breast-feeding on cardiovascular issues," she said.
Another study in the same issue of the journal found, not surprisingly, that American hospitals designated as "Baby Friendly" by the World Health Organization (WHO) and the United Nations Children's Fund had higher breast-feeding rates than other hospitals. These hospitals follow WHO's "Ten Steps to Successful Breast-feeding."
At Baby Friendly institutions, the rate of women beginning breast-feeding was 83.8 percent, versus 69.5 percent nationally. The initiation rate at hospitals with a higher proportion of black patients was only 70.7 percent.
The overall rate of women who breast-fed exclusively during their hospital stay was 78.4 percent at Baby Friendly hospitals compared with a national mean of 46.3 percent.

Words really can't express how grateful I am to be part of such a supportive, thoughtful and caring group of women, and how much I appreciate your beautiful gesture. I will take great care of them and gladly pass them on as soon as someone else is in need.
 
I'm also blown away that you've "scholarshipped" me into the Penny Simkin workshop. Your generosity knows no bounds! With any luck, our situation will change sooner rather than later, and if it does I will gladly pay my way.

Interesting observation on squatting during labor, written by a midwife from The Netherlands. Found it in the Midwifery Today newsletter. I wonder if any of you practice it:
 
 
On the subject of the wisdom of squatting during pregnancy [Issue 7:25]:

As a midwife experienced in assisting some 4000 births, most of them vertically, I am very enthusiastic about squatting whether before or after labor has started. However, there can be a hitch. First of all we need to differentiate between squatting with the heels being pulled up and squatting with the foot soles making full contact with the floor. The latter position means squatting more deeply. This allows the abdominal contents to come down and gives rise to greater pressure being exerted onto the uterus from the thorax for instance. When the baby's head is not yet engaged in the pelvic inlet, the membranes may rupture prematurely and consequently a prolapsed cord can develop. Again I am talking from experience. Therefore, I do not recommend deep squatting during the last two months of pregnancy if the head is not engaged. During labor deep squatting means opening the pelvic outlet. If the baby's head is still high in the birth canal, no form of squatting is appropriate at that moment. Opening of the pelvic outlet goes at the expense of the measurements of the pelvic inlet. The pelvis is involved in a scissor-like movement depending on the hip joints being either stretched like in a standing position or flexed as during squatting. If the presenting part is above the pelvic floor, meaning that the pelvic inlet is still involved, fetal distress can be brought on by squatting.

However, when at the start of the second stage the woman takes a standing position, labor continues in a wonderfully relaxed manner. She need not push actively, because the baby's weight takes the child down. So she does not get tired. Once the baby's head touches the pelvic floor, the woman feels the need to push, and nobody can stop her from bearing down and ending in a squatting position. And when the baby's forehead is born, most women lean backward, thus allowing the pelvic inlet to widen to allow the shoulders to pass. I call this the ballet of the second stage.

I wanted to send out a HUGE thanks to all of you.  I am now the keeper of the blessingway necklace.  Melissa was kind enough to bring it to me last night.  I had forgotten all about it but when I saw I was reminded of all of the friends and well wishes the necklace is connected to.  Of course it brought tears to my eyes.  Debbie had it last and she said it helped her so much to get through her tough time, and I'm hoping it will do the same for me. 

My husband had an accident Saturday night.  He was painting at the new house we are building and lost his footing, falling about 15 ft.  It took him an hour to crawl to the cell phone which was 10 feet away.  He called me and I called the ambulance.  He was taken to Westchester Medical center in Valhalla because they have the trauma center.  He ended up breaking his lower right leg and one of the vertebrae in his back.  Thankfully the spinal cord was not damaged, and he still has all feeling and movement in his arms and legs.  He is getting a body brace today and if it keeps his spine aligned than he won't need surgery. He will have to be in the brace for 6 - 12 weeks, as well as have the cast on his leg from his ankle to the top of his thigh for 3 months.  Then there is rehab and physical therapy afterwards.  He has a long road ahead of him, but all in all, he should make a full recovery.  
Please keep him in your thoughts and prayers.
