If you are needing an endocrinologist because of growth issues, Madeline Harbison at Mount Sinai.  For Diabetes, I am not sure but I would still check out the Endocrinologists at Mount Sinai.

Hi Everyone,

3 questions:

I have a 3.5yo female patient with a head cold- goopy eye,and sinus congestion. 
Her ears look a bit red though she's not complaining of any pain.  The strange
thing though, is that she had some bright white granular powder (looked like
small bits of styrofoam) in both left and right ear canals.  Not a ton of ear
wax, but there was a bit of ear wax on the left looked red (like ketchup).  Her
mom couldn't think of anything that might have gotten in her ears, and she
hasn't been seem putting things in her ears either.  Sheh as a history of
occasional, mild ear infections.

Any thoughts on the white granules?  Is reddish wax just old wax?


Finally, can anyone recommend a good photo reference website for
ears/infections?

I once saw a boy who had a plastic bead lodged in each ear canal and denied putting them there. Reddish earwax is normal. If you google eardrum or otitis in images you will find lots of pictures.

It also may be ear eczema debris.  If the child has a hx of ear irritation, could be a lot of itching/scratching going on.  I've looked at a lot of kids' ears, and I see ear wax ranging in color from translucent to deep orange (red?).  Fungi are often overlooked as a cause of ear pathology, and while I haven't seen white, I have seen black.  So consider fungus as well--best confirmed via culture.

ANd of course, she's 3.5.  It IS possible she has been putting something in her ears without mother noticing.  I assume she has not been candled?  That would cause a debris almost like what you describe.

I have seen dry ear wax look like white debris in the ear especially in younger
children.
If you see white cauliflower lesions protruding from the eardrum or in the ear
canal, you should rule out cholesteatoma (although uncommon, it is one of the
complications associated with recurrent ear infections).

Cholesteatoma is a destructive and expanding keratinizing squamous epithelium in
the middle ear and/or mastoid process.

No, the white bits were definitely not on the ear drum, just the ear canal.  It looked like the inside of a snow globe in her canal... Most likely some foreign objects that either floated in there or were put in there.  Thanks everyone for your feedback... I just wanted to be sure I wasn't missing a strange and unusual illness.  :)

Dear Dr. Randall:

I am trying to see if you can give me some advice on what to do or where to seek for help. I used to live in the Bay area and got to see the presentation of your book in Palo Alto. I have a son who is 2 and 1/2 year old aparently very healthy never has taken antibiotics neither has him being vaccinated. He got a stomach flu in december 19 had fever for a day then he developed flu and around the beginning of January he had cough with phlegm that the doctor said was bronquitis. My husband and I decided to use natural remedies for his recovery and within 7 days of using colloidal silver, licorice, and the clear lungs supplement  he seemed to have overcome the cough and phlegm. Well since two weeks ago he has been telling about something burning his stomach. He rolls up his pants and sleeves at night and tells me that he need cold. He also seems to have lots of liquid on his mouth that seems sour and makes him scratch his nose a lot. He looks slimmer for his usual body but does not seem to have lost weight. He is pale and has lots of overstimulated energy but seem tired compared with how active he was outdoors. He complains of tiredness after a little bit of a walk. He is suffering from something but neither the specialist, regular pediatrician or ER can figure out what. 2 night ago in the ER the doctor tested my son for Leukemia and results were negative. The doctor advice to speak with another pediatrician and I am just frustrated and tired of  not finding any relief for my son's restless, painful nights. Unfortunately I do not leave in the Bay area I am in Orange county. I cannot find a holistic pediatric in the area so I was wondering if you can advise me into what to do. I have given my son a natural antacid but he seems to still have that liquid that wakes him up at night. He also does not want to nap. We have change his diet to small meals, raised his bed crib but nothing seems to be working. I am desperate for some answers.

Ryoko Takayama http://www.familywellnessacupuncture.com  is a great resource in Orange County.  Here's a little of her bio, and I believe she specialized in Pediatrics in Japan, and she is a Holistic Pediatric Association member/practitioner.

Ryoko Takayama (Ree-Yo-Ko) is a highly educated and trained acupuncturist. She began her education
at the Meji University of Oriental Medicine in Kyoto Japan; the first college in Japan to offer a bachelor's
degree in the field of Acupuncture. She received her Japan Acupuncture License in 1995 and her
Bachelor of Arts in Acupuncture and Moxibustion in 1996. Ryoko worked as an acupuncturist in several
hospitals in Japan over the next 5 years; seeing as many as 30 patients a day.

In 2001, Ryoko emigrated to the United States to practice Acupuncture. She entered Acupuncture and
Integrative Medicine College in Berkeley to begin her American Acupuncture studies and accreditation.

Ryoko Takayama (Ree-Yo-Ko) is a highly educated and trained acupuncturist. She began her education
at the Meji University of Oriental Medicine in Kyoto Japan; the first college in Japan to offer a bachelor's
degree in the field of Acupuncture. She received her Japan Acupuncture License in 1995 and her
Bachelor of Arts in Acupuncture and Moxibustion in 1996. Ryoko worked as an acupuncturist in several
hospitals in Japan over the next 5 years; seeing as many as 30 patients a day.

In 2001, Ryoko emigrated to the United States to practice Acupuncture. She entered Acupuncture and
Integrative Medicine College in Berkeley to begin her American Acupuncture studies and accreditation.
While in school here, she was employed by AIMC Berkeley in varying capacities until receiving her
Master of Science of Oriental Medicine (MSOM) and acquiring her California Acupuncture license in
2005. After receiving her Acupuncture license in 2005, Ryoko continued to work with AIMC Berkeley and
quickly became the clinic manager there.

I am trying to see if you can give me some advice on what to do or where to seek for help. I used to live in the Bay area and got to see the presentation of your book in Palo Alto. I have a son who is 2 and 1/2 year old aparently very healthy never has taken antibiotics neither has him being vaccinated. He got a stomach flu in december 19 had fever for a day then he developed flu and around the beginning of January he had cough with phlegm that the doctor said was bronquitis. My husband and I decided to use natural remedies for his recovery and within 7 days of using colloidal silver, licorice, and the clear lungs supplement  he seemed to have overcome the cough and phlegm. Well since two weeks ago he has been telling about something burning his stomach. He rolls up his pants and sleeves at night and tells me that he need cold. He also seems to have lots of liquid on his mouth that seems sour and makes him scratch his nose a lot. He looks slimmer for his usual body but does not seem to have lost weight. He is pale and has lots of overstimulated energy but seem tired compared with how active he was outdoors. He complains of tiredness after a little bit of a walk. He is suffering from something but neither the specialist, regular pediatrician or ER can figure out what. 2 night ago in the ER the doctor tested my son for Leukemia and results were negative. The doctor advice to speak with another pediatrician and I am just frustrated and tired of  not finding any relief for my son's restless, painful nights. Unfortunately I do not leave in the Bay area I am in Orange county. I cannot find a holistic pediatric in the area so I was wondering if you can advise me into what to do. I have given my son a natural antacid but he seems to still have that liquid that wakes him up at night. He also does not want to nap. We have change his diet to small meals, raised his bed crib but nothing seems to be working. I am desperate for some answers.

What type of results have you found and what is the standard dosage range? I
remember it being mentioned in a lecture at the IFM symposium but can't seem to

Shonishin (children's needle therapy) is a unique, non-invasive and gentle pediatric style of acupuncture that developed over 250 years ago in Japan. Safe, quick and easy to do, these modified Japanese acupuncture methods are well received by babies and children. Shonishin effectively treats numerous conditions common in childhood and fortifies children's vital energy, thereby helping strengthen their constitution and maintain children's health.  It usually involves the application of light touch diagnosis and simple treatment patterns that involve non-insertive techniques such as rhythmic stroking, pressing, scratching, tapping, and contact needle acupuncture. Parents, children and practitioners like shonishin because it is not painful and is thus not scary to the child. Modified forms of it can also be taught to parents for home therapy to augment the treatment process. In this seminar will be a combination of lecture, demonstration and practice of these practical, simple and effective tools that acupuncturists can rapidly implement in their existing practices.

I just saw a 14 month old girl who was diagnosed with bilateral bladder reflux
after developing a kidney infection at the age of 8 months. She is being treated
prophylactically with novo trimel 2.5 ml/day to prevent further infections. Mom
has been told there is a 50 - 60% chance of her outgrowing the reflux. She has
eczema and cradle cap (not sure when that started) and uses a moisturizing
non-medicated cream daily for that. She broke out in hives last month after
eating peanut butter for the first time and saw an allergist who did the sratch
test. She is only allergic (on the milder side) to peanuts, but is now avoiding
all tree nuts.
She has been fully vaccinated, was breast fed until 8 months and then put on
Nestle Good Start wtih omega 3 & 6, eats all foods and drinks milk daily.
Vaginal non-medicated birth.
I have recommended an infant or children's probiotic daily while she is on the
antibiotics and gave her fermented cod liver oil to take daily. The mom was
hesitant to take her off all dairy until she plans to wean her from her bottle
at 18 months but was agreeable to trying her on goat's milk products for a month
to see if the eczema improves with that. I have instructed the mother in how to
massage the kidney, stomach and lung channels for support. I have also
recommended avoiding all refined flours and sugars, and using sprouted grains
whenever possible.
her shen is very good, there are no dark circles under her eyes or veins
anywhere on her face. She is not a picky eater and loves to be touched. Bowels
are twice a day and well-formed. Sleep is good. She is generally very happy and
engaged.
My question is about the reflux which I think was the original cause of most of
the issues. Is there anything that can be done herbally to help with that? Would
it be worthwhile to give her a formula for the eczema or is it reasonable to
first wait to see if changing and then stopping dairy helps? I am trying not to
overload the mom.

Depending on what degree of reflux (which they should know by now from the xrays) there are two options. One is to take antibiotics, as she is doing, the other is to put her on some antibacterial herbs (such as the Evergreen Herbal ABX formula or similiar liquid extract) and do periodic urine tests every two to three months (either dipsticks or cultures) to check for bacteria. This may not be feasible if she's not potty trained yet. Constant probiotics. And I would give her a kidney tonification formula to see if you can encourange progress in the the urinary tract development. Treating the underlying allergic component would be good as well with the protocols set out in my book The Holistic Baby Guide.
