HI--I started on Atripla 3 weeks ago and have had NO side effects whatsoever.  I
guess I"m lucky in that regards. The one thing about the pill is that one of the
ingredients will cause you to fail a drug test (apparently its similar to pot)
so if you have to take a test be sure you have a doctors note.

   I also take the pill at night before I go to bed.  I have another blood test
coming up in a couple of weeks and it will be interesting to see what Atripla
has done to my t-cell and viral load counts.

The only bad part of Atripla is the cost--$2,000 per month and I'm on Medicare
so I'm in and out of the donut hole quicker than you can say Jack Rabbit.

schedudle so you don't miss any meds as atripla isn't forgiving and you don't
want to become ristant,,,and be sure to drink plently of water,,you need to wash
the meds into your system and wash them out of your liver where they can cause
harm.......lote of water i cann't stress that enough,,if you don't like water
invest into some sort of filter,,i have a reverse osmossis and a britta pitcher
i keep refrigrated,,,keep strong....mike

Does anyone have specific info on what insurance plans in New York City/State
offer the best deal for Atripla?

I'm a NY state public employee, undetectable 11 years on an old treatment regime
(Norvir, Invirase, DDI) with minimal side-effects. My doctor is recommending
Atripula so I can take 1 pill a day instead of the several that I've been.

But my current insurance, Aetna, prices it as a "tier-three" medication that
would cost me a co-pay of literary hundreds per month. I have a long list of
other insurance carries from my job that I can switch to come this October but
have no idea  which offers the best deal for hiv meds in New York. My income
disqualifies me for ADAP or any other public help. Does anyone have info on
which insurance is best for these newer treatments? Many thanx.

Hi everyone,  I'm a 54 year old man with Aids and I have been on disability or
several years.  I have no assets other than clothes and a few other personal
belongings.  My partner of over 3 decades owns a house, cars, stock, cd's, etc. 
He wants to set up a trust for me so that in the event that he dies, I'll
receive the benefits of his estate without compromising my benefits...ie, med
assistance, prescription help, etc. He has consulted an attorney but I'd like to
talk to someone who is involved with a trust to see if they'd do things
differently if they could redo the trust.

I think we all know how difficult, even frightening disclosure can be. But if
doing the right thing was always easy, everybody would. The article posted is
extremely misleading, included some outrageous excesses by legal authorities
conflated with appallingly unethical behaviors by some poz individuals. The BBC
for instance reported that the German pop singer cited in that article did
indeed have unprotected sex with a partner without disclosing her status that
resulted in that partner's infection.

Contrary to this article's deliberately misleading and hysterical title, HIV+
people are not being criminalized for having sex or even for having unprotected
sex. What's has been and should be prosecuted in some cases is deliberating
putting others at risk through lying or withholding information about one's
status. People should be free to have all the sex they want with other
consenting adults. The operative words are consenting adults, and lying through
omission or commission is to deliberately deprive others of their right to
consent. Misleading headlines or individual cases of excess or abuse of
prosecutoral discretion, even if motivated by fear and intolerance of HIV+
people, cannot obscure the fundamental ethical and moral issues here. We have to
strive to overcome our own fears of stigma, rejection and self-blame to do what
we know to be right and fair to others. I know first hand how this is often
easier said than do. But to reject that
  principle, even if its imperfectly applied at times, is to led HIV+ people into
the abyss of nihilism, self-hatred and sociopathic, predatory behaviors. We must
do this first and foremost for ourselves, not only for our partners.

I'm definitely in favor of some form of heavy duty legal retribution for
nondisclosure. I don't understand how anyone could have a soul or a conscience
and still infect someone with a deadly virus. I guess I'm pretty naive.
Personally I've not had one person turn me down for sex after disclosure and
several times he was HIV+ himself...which makes me wonder if and when he was
going to tell me his status.
 
There is another side to the coin these days.  HIV is quickly becoming a
chronic disease and not the death sentence it used to be. So here's my
question.  What about other infectious chronic diseases?  Herpes, STD,
pneumonia, tuberculosis, etc.? Where do you draw the line.

Obama administration launches a sex-ed program

Over the past decade, politicians have battled about how to reduce the teen
pregnancy rate: safe-sex vs. abstinence-only sex education programs, even as
films such as "Juno" and births by famous teens such as Bristol Palin and Jamie
Lynn Spears seemed to make adolescent pregnancies more socially acceptable.
At the same time, after declining for years, the teen pregnancy rate increased,
but the pace at which teens were having babies appeared to stop falling or even
inch up.
Now, the Obama administration has entered the politically sensitive debate,
promising to put scientific evidence before political ideology. A $110 million
campaign will support a range of programs, including those that teach about the
risks of specific sexual activities and the benefits of contraception and others
that focus primarily on encouraging teens to delay sex.
The initiative exemplifies the administration's oft-repeated quest to find new
strategies to defuse some of the nation's most divisive issues. In this case,
officials are hoping to appease advocates of teaching teens about condoms and
other forms of birth control as well as those who oppose sex outside marriage.
Although the program is being hailed by many adolescent health experts, it is
being denounced by some on both sides of the abstinence debate.
"This is one of those emotionally charged issues where it's very difficult to
find compromise," said Amy E. Black, a political scientist at Wheaton College in
Illinois. "It inevitably becomes entangled in a larger constellation of issues,
such as abortion, that raise ideological, moral and religious questions."
During the George W. Bush administration, the federal government spent $1.5
billion on programs that encouraged teens to delay sex until marriage. Critics
said it was grounded in religious tenets and conservative doctrine, failed to
educate teens about condoms in the age of HIV and other sexually transmitted
diseases, and ineffective.
In response, the Obama administration launched a teen pregnancy prevention
program that officials promised would fund only programs that had been proven to
work. Last month, the Department of Health and Human Services awarded $75
million to 75 groups to try to reproduce some of the 28 programs deemed to have
been "proven effective through rigorous evaluation."
HHS also awarded $35 million to 40 organizations to test "innovative strategies"
that appeared promising. Altogether, 115 programs in 38 states and the District
received funding.
"We were hoping for and hopefully got a healthy mix that will be both spreading
evidenced-based programs around the country and allow us to identify programs
that are as effective or more effective," said Sharon Parrott, human services
counselor for HHS Secretary Kathleen Sebelius.
Many experts agree.
"What's exciting and innovative about that is not only the full-fledged return
of science to the field of teenage pregnancy prevention but also the opportunity
to adapt these approaches to the needs of individual communities," said Michael
Resnick, who studies adolescent health issues at the University of Minnesota.
But after studying the programs that won funding, critics of abstinence programs
expressed dismay at the inclusion of curricula they consider discredited. Twelve
grants totalling more than $9.3 million went to abstinence programs, according
to HHS.
"They are funding programs that censor information about condoms and birth
control and have elements that are clearly ideological and not science-based,"
said James Wagoner of Advocates for Youth, a Washington advocacy group. For
example, Live the Life Ministries of Tallahassee received $891,533 to try the
WAIT Training abstinence program on 5,500 students in middle and high schools in
14 Florida counties.
"The problem is that this program is ineffective and withholds important
information from young people," said Monica Rodriguez, head of the Sexuality
Information and Education Council of the United States, a nonprofit research and
advocacy group. "There is very little information about condoms or other forms
of contraception, and what information is there is biased and skewed towards
portraying them in a negative light."
Wagoner and others also criticized a $933,907 grant to Lighthouse Outreach in
Hampton, Va., which plans to use the Choosing the Best curriculum to target
2,600 youths ages 10 to 19. Critics say the program includes misleading
information about condoms and asks teens to make "virginity pledges."
Wagoner and others expressed concern about some of the more than $1 million
awarded to the Women's Clinic of Kansas City, Mo. The clinic is one of the
"pregnancy crisis centers" that have been denounced by reproductive rights
supporters who say they proselytize to pregnant women about the evils of
abortion.
Abstinence proponents said they were disappointed by the small number of
abstinence programs that received funding, saying they had identified just five
"authentic" abstinence programs receiving less than $5 million. Under the Bush
administration, 169 abstinence programs were funded to serve more than 1 million
students, and those programs face extinction, said Valerie Huber of the National
Abstinence Education Association.
"We're very disappointed," she said. "Obviously, this policy change has taken
away all emphasis on avoidance. A paltry number of abstinence programs were
funded, neither giving priority to a risk-avoidance message nor giving equality
to this message."
Officials at some of the groups singled out by critics defended their programs,
saying that they were required to provide "medically accurate" information about
contraception and that it would be carefully evaluated by independent
researchers.
"The goal is to come up with a road map for your life. What are your hopes and
dreams? And how might sexual activity derail that?" said Richard Albertson, who
heads Live the Life Ministries. "We want you to do things in the right order:
Finish your education, postpone sexual activity until marriage, and then don't
get married when you are a teenager."
"Our program is not going to be pro-choice or pro-life," said Deborah Neel,
executive director of the Women's Clinic. "If they come into our center, we
don't say abortion is not an option. That's stupid, because abortion is legal.
But the thing is, you have to provide abortion education and abortion
alternatives. You have to show the full range of choices."
Joyce Richardson, project director for Lighthouse Outreach, said that the
program does not teach students how to use condoms or other forms of
contraception but focuses on how often they fail. But she said the information
is balanced and accurate.
"We do teach them about the effectiveness rate, or lack thereof," she said.
Parrott said that the programs trying to replicate proven curricula are required
to follow the approved courses and that those trying new approaches must undergo
independent evaluation.
"I think we have the appropriate oversight in place," she said. "I think this is
an opportunity for a broad range of programs to attack a really important
problem."

some older anti-HIV agents have been associated with lipoatrophy,
specifically changes in the appearance of the face. Most anti-HIV drugs used
today are not associated with this problem. However, for people in whom
these problems have occurred, facial fillers may be an option. One such
filler, Bio Alcamid (polyalkylimide), is considered a permanent implant.
Reports over the past several years suggest a greater-than-expected risk of
complications in people who have received this product.
*About Bio Alcamid*

Bio Alcamid is 96% water and 4% gel (polyalkylimide) and resembles fatty
tissue. It is implanted just under the skin. After surgery, the body
surrounds the product with a network or capsule of collagen. This
encapsulation is supposed to restrict the bodys exposure to Bio Alcamid and
prevent it from moving (migrating) to other parts of the body. According to
doctors from the Netherlands, early literature on Bio Alcamid described it
as safe and possible complications such as infections were supposed to be
rare (about 0.06%).
*NetherlandsA large study*

In the past several years, reports have emerged of higher-than-expected
rates of complications with Bio Alcamid. Members of the Dutch Society of
Cosmetic Medicine conducted a survey of plastic surgeons to find out some of
the issues associated with Bio Alcamid. They found that 3,196 patients
received 4,738 treatments with this product. Of the 3,196 patients, nearly
5% developed complications, which could be broadly grouped as follows:

    - Inflammation  This was the most common problem occurring years after
    implantation and mainly after surgical procedures in the face, such as
    dental procedures, noted the Dutch doctors. Inflammation associated with
    the implant also occurred after patients experienced infections in their
    head and neck. However, in about 50% of patients no obvious cause of
    infection could be found.
    - Accumulation  In some cases, Bio Alcamid seemed to build up in one
    area, typically years after [implantation], according to the doctors.
    - Hardening of the capsule  Another complication was hardening of the
    capsule of collagen surrounding the implant. The doctors stated that this
    was generally a visible and disturbing disfigurement for patients. No
    obvious reason for this problem could be found.
    - Migration  In some cases, the implant would move to another area. This
    was a problem particularly in the cheeks.
    - In HIV-positive people  As part of their larger study, the Dutch
    doctors also collected data on a sub-group of 270 HIV-positive people who
    received implants of Bio Alcamid. Overall, the types of complications seen
    among HIV-positive people were similar to those seen in HIV-negative people.
    HIV-positive people had slightly more complications (6%) than HIV-negative
    people (5%).

Ideally, fillers used for reconstructive surgery should be as safe as
possible, stated the Dutch doctors. However, the complication rates seen
with Bio Alcamid are, in their opinion, too high. Furthermore, these
doctors note that their analysis of the risk-benefit ratio of Bio Alcamid
takes into account the health risk of inflammation and the difficulty
treating disfiguring complications. The Dutch Society of Cosmetic Medicine
now advises its members to not use Bio Alcamid.
*ScotlandSome changing views*

Some plastic surgeons in Edinburgh initially had a favourable impression of
Bio Alcamid based on short-term results that they saw. However, now these
plastic surgeons have found a high rate of complications appearing after
implantation of Bio Alcamid. Some patients even needed intravenous
antibiotics along with surgical drainage and further corrective surgery to
recover from their complications. The Edinburgh plastic surgeons said that
Bio Alcamid may have significant long-term problems and its use should be
restricted or discontinued entirely.
*IsraelUnexpected complications*

Doctors in the Department of Plastic and Reconstructive Surgery at the
Sackler School of Medicine at Tel Aviv University in Israel have also
unexpectedly found complications among 14 HIV-negative people who received
Bio Alcamid. These doctors found that serious complications occurred in
patients without [a history] of trauma or oral [surgery]. The Israeli
doctors theorize that perhaps Bio Alcamid may somehow interfere with
surrounding tissue, making such tissue prone to infection.

Like several other teams of plastic surgeons, the Israeli doctors found that
removing Bio Alcamid that had been implanted for a long period of time was
not easy. Traditionally, cuts to the face for drainage and removal of
infected tissue and material might be one approach for dealing with
complications related to Bio Alcamid. However, this carries the risk of
leaving permanent scars. The Israeli plastic surgeons appear to have
developed a novel mini-irrigation system that helps make the removal of Bio
Alcamid easier.
*CanadaUnexpected long-term complications*

Infectious disease specialists at Torontos General Hospital have been
struggling to help HIV-positive patients who have infectious complications
arising from Bio Alcamid implants. Also, other Canadian doctors plan to
publish their experience with complications from Bio Alcamid.
*Caution about permanent fillers*

The complications seen in some users of Bio Alcamid may also occur with some
other facial fillers that are considered permanent implants. Part of the
problem with evaluating the safety of facial fillers is that the regulation
of these products differs from one country to another.

The International Committee for Quality Assurance, Medical Technologies and
Devices in Plastic Surgery (IQUAM) has made this important point in one of
its reports:

*All *permanent soft tissue fillers are associated with risks of infections
and granuloma formation, which may lead to major disfiguration. This risk of
infection depends on many factors, including the following:

    - the nature of the soft tissue filler
    - the amount of filler used
    - the depth of injection
    - the part of the body being injected

Plastic surgeons who have reviewed the IQUAM report note that even when
products are implanted by very skilled plastic surgeons, the risk of
complications can never be ruled out.

IQUAM has found the following:

Substantial biochemical and biophysical differences exist between
commercially available soft tissue fillers as well as variations in their
purity. Clinical studies on new permanent soft tissue fillers offered by
manufacturers are mostly not sufficient to predict the incidence of late
on-set reactions.

Perhaps the most important point made by IQUAM is this:

Most [permanent] soft tissue fillers have not stood the test of time and
thus should still be considered experimental.

Bearing all of this information in mind, we encourage HIV-positive people
who have had permanent soft tissue fillers implanted and who are concerned
about complications to discuss any issues with their physician and, if
necessary, obtain a referral from their physician for an experienced plastic
surgeon.
*Statements *

As mentioned earlier, the Dutch Society of Cosmetic Medicine now advises its
members to not use Bio Alcamid. The Swiss Society for Plastic Surgery has
banned the use of permanent soft tissue fillers. Furthermore, the Secretary
of the International Confederation for Plastic Reconstructive and Aesthetic
Surgery (IPRAS) has issued a warning about the complications that may arise
from the use of permanent facial fillers.

I'm surprised by the acidity of the response to this piece.  And to the original
video they are discussing, for that matter.  But I get it: one mustn't appear
the least bit flip when riding the rails between praising negative guys -- and
being accused of insulting the positive ones.

But I believe the issue is out there, and that being, the resentments on both
sides. I regret my style got in the way of my substance.

Is there currently anyone doing AIDS/HIV chaplaincy in San Diego?  I recently
had a call for a referral and I don't know of a clergy person who takes this on.
No urgency right now but any information is welcome.
