The ongoing and long term management of AF in primary care – How GP’s agree a management plan with patient, how they handle it and what is involved? What recourse to specialist input and ongoing testing is offered and what is needed and why?

Or, i s long term management of AF best provided by arrhythmia specialists (EPs and/or cardiologists)? If so, is this precluded at present for many people due to lack of resources and if so, what can be done to improve matters: a) in short term and pragmatically by making available testing/monitoring resources more accessible to GPs and b) in long term by training and recruiting more EPs?
These aspects were not really covered last Sunday and rather than rehash some that were I think this area would be suitable as it is the pathway to many aspects of treatment as AF progresses/treatments improve and would hopefully illustrate how patients do and could 'tap' into the available resources through GP level entry. If patients slip through the crack between good GP monitoring/management and specialist care they risk being 'lost' in the system and when that happens the AF can worsen for lack of appropriate monitoring and attention, no matter how good the latest techniques and medicines are.
