I called the Irlen Institute and got a copy of their petition. It
is much simpler than mine, but it includes a little more information
about the signers. Here a copy.

Even if you can only add your own signature and possibly the
signatures of your family members, it would help enormously. To
streamline the contact process, one quick email could be composed
stating your opposition to the proposed ban and sent cc to each of
the legislators.
