Dear FAP therapists and researchers,

In October we asked FAP therapists/researchers/students to participate in a
study that explored individuals’ attitudes towards people with mental illness,
their specific attitudes towards people diagnosed with two different mental
illnesses, individuals diagnosed with a Substance Use Disorder (i.e., Substance
Abuse or Substance Dependence) and individuals diagnosed with Major Depression,
and their attitudes towards in-vivo interventions. We appreciate your
willingness and time to participate in that study.

Based on the results of that study and participant feedback, we have revised the
study and are now asking for FAP therapists/researchers/students to participate
in our revised study on attitudes about individuals with mental illness,
Substance Use Disorders, and Major Depression, as well as their attitudes about
in-vivo interventions. If you participated in the October study, you ARE
ELIGIBLE to participate in the current study. This study has received approval
by the University of Washington Human Subjects (32982). Your participation in
this survey is completely voluntary. The study will involve an on-line web
survey that will take about 45 minutes of your time. As compensation for your
participation, you will have the option to enter your contact information in a
drawing for one of three $50.00 gift certificates to Barnes and Nobles. You
will not be asked to give your name at any time
except if you choose to enter the drawing for the gift certificates. Your
contact information will be entered onto a secure webpage that is separate from
the study. At no time will your contact information be associated with your
responses given in the study.

You will be asked for your demographic information, but the raw data will not be
seen by the primary investigators. A research assistant unfamiliar with members
of the FAP community will be responsible for the raw demographic data. This
research assistant has undergone training on maintaining participant
confidentiality and has signed a confidentiality agreement. You will be asked
for your opinions about individuals with mental illness, with Substance
Abuse/Dependence, and with Major Depression. You will also be asked about your
opinions about a specific component of FAP therapy, the in-vivo process. You
will be asked to complete two cognitive tests. These tests will ask you to
respond quickly to images and words flashed on the computer screen. At the end
of the study, you will be asked to provide feedback about the study’s measures
and procedures. There will be a “frequently asked questions” section to
provide you with further information at the end of survey.

The survey is managed on a University of Washington and Web Inquisit web servers
that use the same secure transfer protocol used by banks and other commercial
websites. Data that you provide on the study website are protected in
transmission by SSL encryption. No personally identifying information will be
collected unless you choose to enter the drawing for the certificate. This
information will be collected on a webpage separate from the study, and this
webpage will be managed on a University of Washington web server with the same
security as the study website. Your name will never be associated with the data
you enter at the study web address.

This survey has been designed to be as convenient for participants as possible.
The survey can be accessed from any PC format/computer 24 hours per day/7days
per week. Although all security protocols have been implemented, as for all
web-based interactions, complete confidentiality cannot be assured. You will
not be asked for your name. You may end your participation at any time by
exiting your browser.

We strongly encourage women and minority FAP community members to participate.

There is no direct benefit to you for participating in this survey however, you
will be contributing to scientific knowledge through your participation.
