Phillip's OT was just here and recommended fruit leather for him to
use for chewing practice.  She recommended getting it at Trader Joe's
or Whole Foods.

child increases or learns to lateralize the food.  My daughter just kept her
food in the middle and eventually spit it out forward or gagged on it if it
went backwards.  This seemed to help.  I was also suggested to use beef
jerky or somethign that wouldn't break off for the kids to gnaw on for
practice and get their salivation going or something.  She also suggested
big pretzel rods wtih fruit rollups wrapped around them to gnaw on...but our
always broke or she played with them and hit them and they broke.  We found
that some fruit roll ups contain a lot of dyes as my other kids ate them
when she didn't like them and that therapy idea didn't work for us...their
poop ended up bright green or blue or whatever after that! Made diaper
changing exciting for a day or so.  Hope that helps.  I would check with a
therapist and if they seem clueless, maybe check with a new one.  I have
found some to just observe and learn from our kids to get their experience
and others to be way experienced in this stuff.  I just keeping switching
therapists in my early intervention program to get one that knows more.

Hi Laura,
My daughter Raven (now 3yrs) has GERD, severe sensory issues and low muscle tone. Up until last Sept she was living on baby food puree and whole milk fortified with whole milk powder and carnation instant breakfast milk. In July I began taking her to see a behavioral feeding therapist to address food fears and get her to try new things. At that point she had no chewing skills. Her SLP had been doing oral motor with her but she was afraid of a lot of things (chewy tubes, electric toothbrush). In august I went to a conference by Lori Overland "Feeding: A sensory motor approach. (at Talktoolstm.com they have all their conferences posted). This conference was amazing it was geared for SLPs and OTs, but parents were welcome (I was the only parent who was not also an OT or SLP). At tis conf I learned the mechanics of chewing in a detailed way. I learned why my daughter couldn't handle foods as she couldn't chew as Lori had us put stuff on our tongues and we were not allowed to chew and to feel what that felt like. Over the 2 days I learned how to recognize various different problems, then to make a treatment plan to address them and to carry it out. It was very hands on and there were 2 demonstrations with 2 different children Lori had never met. One thing I really liked about her was her support for parents and our issues of having to do the nutritionnal feeding vs therapeutic feeding and also her developing intervention techniques that utilize things parents will do anyway such as toothbrushing as she knows we are busy! I learned so much. After the conference I immediately got Raven off the sippy cup as apparently this is just like nursing and doesn't support growth of oral motor skills. They use straws to strengthen oral motor and I switched Raven to only straw cups and the Playtex drink up cup. I started doing alot of bubble blowing with Raven (I bought blowers from talk tools that she could use as she couldnt use conventional bubble blowers). Raven was wiling then to do oral stim with an electric toothbrush.  Also the SLP and I started using the Chewy Tubes with her. The most amazing thing is that she learned to chew by mid November! She actually eats choc covered graham crackers! Of course that and her mushy but no longer pureed food is all she will eat but I am still working w/ the beh feeding therapist to increase her repertoire!
I would advise you to check out the Talktools site and see if the conf wil be held anywhere you can get to- it is so worthwhile!
Also make sure your OT and SLP are expert at oral motor if not try to look for someone who is or who was trained by Lori (She does her conferences all over).

Wow...I apologize if I came across negatively!  I surely didn't mean
it that way!!!! : )  It is helpful to have a chart like this one.  I
guess after 5 months of seeing my baby at the under 3rd percentile for
weight, struggling with BPD, cerebral palsy, and aspiration
issues...trying to "force" food whenever possible just to get him to
take "enough" calories, and trying to figure out what the heck
"enough" is...I guess I just thought there may be a couple of parents
who could be discouraged by the figures.  Sorry for the upset!

Wendy, I am not aware of anything like that, but I am sure you could write
it off at the end of the year as a medical expense...medical training,
especially if your ped or GI wrote it down on their letterhead or Rx pad.
You would also probably be able to write off the mileage and parking to the
conference.  I know my GI's nutritionist wrote down everything on a little
notepad with their letterhead/business logo on it (like a big Rx pad)
including hte date, her signature, and all the recommendations such has
whole milke with formula, vege oil/ olive oil, purees, carnation instant
breakfast or  pediasure. It is medically necessary and considered
over-the-counter drugs and the date on the "order" could be compared to the
child's birthdate in an audit and show that the child is way past the time
to continue taking this stuff age wise and therefore back up the MD's
suggestion to continue offering these OTC's for necessary weight gain, etc.
.  I was at my accountants office today and forgot to ask this stuff.  Now
that it is tax season, maybe someone could ask their accountant about this.

  Thanks for the offer... any questions that you feel that would be appropriate for asking of this drug being used on a 2 yr old (27mth's) for behavioral issues would be greatly appreciated and then you could just let me know any interesting or fact's about it. I would greatly appreciate it! I did do a lil research on it this morning and as I had thought it's used for psychiatric/behavioral issues and according to the info has been commonly used in children for more than 10 yr's.... however there wasn't an age range though so that wasn't very helpful. I guess I could give you the dosage amount and you could mention that along with age just to see what opinion they have to offer. It's a very small dosage... The pill is .25 mg once daily before bedtime... but only a 1/2 of pill... (not a whole one) and she's 27 mth's. Any info would be great..... I hate using any kind's of medicine's, I just don't want to keep my dd drugged if not necessary.

Just so you know, none of the psychiatric "behavioral" medications have been approved for children as no-one is doing the studies on children (there was just an article on this in a local magazine in brooklyn, NY). What this means is that no-one knows the dosages,  long or short term outcomes or possible side effects for children (apparently children metabolize drugs faster than adults). The company that makes risperidol is actually doing a clinical trial right now with children. Their rep is coming to do a talk with my staff tomorrow so I will ask her the status of this study and what ages they are studying. This is not to say that you should not consider it for Casey if she really needs it. The FDA allows MD's to prescribe any drug to anyone as long as they believe that it is necesary and will be effective. I would just make sure that you start with a very small dose and that Casey is closely monitored by the MD with frequent visits. Some MD's as we all know are b etter than others. In my work I have encountered both responsible and irresponsible ones. I'm sure that you will be on top of the sitiuation. Please let me know if you have any specific questions I could try to ask the risperidol rep for you.

So as a last resort the only thing I could think of was maybe something going on brain wise that we don't see an d called her neurologists who wasn't far away from the ENT doc. I had to carry on and on to get in b/c it's almost impossible to see him at times but she finally called me back on my cell before leaving town (which is 2 hrs one way from our house) saying that she had a 3:45 cancellation so we ran around a bit then went over there. The doc checked her out, measured her head, asked lot's of question's, and I explained the degree of it and how long it's been going on as well as her sleeping as lil as 3-4 hrs in a 24 hour period at times b/c of it. After checking her out he said he believe's she is experiencing a compulsive obsessive behavioral issue b/c of her brain delay's and using it as a way to express herself and get out her frustration's since she is delayed. So... he want's to try her on a behavioral medicine before bedtime each night to see if it help's... I guess it can't hurt.  I pray this is the solution and it help's and I get my happy lil girl back. I asked him how sur e of this he was and he said he's almost certain but to be on the safe side he would order a MRI but since she's uncooperative she has to be fully sedated and we can't get in until the 15th. Then we see him on the 21rst for a follow up. Anyway's, that was our long and very frustrating day... oh yes, and that was all after not being to bed since 1:30 this morning when my dd decided to wake up screaming with a tantrum after only 2 hours of sleep.

This is what we all expect as well.  However, our son has had the fundo and g-tube surgery!  So no one knows what's really going on there and we have to have several tests done.  Quinn breathes silently when he's sleeping, but he has to work very hard to breathe when he is awake.  It's just a nightmare with the nebulizer, suction machine, chest PT, positioning.  It's awful.

I am going to a feeding evaluation tomorrow and will try to find out more.  There is also a program with some detailed info regarding a feeding program that has weekly visit, or even an intensive 8-5 daily inpatient program that is a full 8 weeks.  See it somewhat described at www.marcus.org and look under topics of feeding program or something like that.  I am hearing from OT's though that it is mostly for behavioral problems and don't deal with much of the sensory, medical issues that relate to some of our feeding problems, but the brochure and description says they do deal with those.  Tehre was a seminar I was to attend this past Sat that got cancelled due to bad weather here in GA so I can't tell you anymore about that specific program.  They are linked to Kennedy Krieger programs if you want to look up that link as well.  It is on the Marcus website as a link as well.

Hi Nicole,

I think it's always helpful to have a baseline that is true for *most*
kids.  Like everything, you have to play around with it to make it
work best for your own child.  I'm printing off a copy of the chart
you sent just to have...

My DS is 19 months old and his primary source of calories is formula
and benecalorie.  He likes to eat solid foods but he doesn't chew it
correctly.  He does receive OT, PT & ST services and the OT does
work on feeding.  However, we don't know what to do to help him chew
better.  He seems to mash stuff with his tongue and has an up & down
chew but not a rotary chew.  I dont' know what to do to help him
learn how to chew his food.  Is there anything I can do to help him?

Niki,
  I did read the side effect's and I really think it's worth a try... so were trying it. It's a very low dosage and early on Casey was on so many different med's that were similar as far as not being tested on kid's but they helped and that's all that mattered at the time. I think they just have to say that to protect their own behind's. I still like to know all the what's if's.

you are going to find this with a lot of drugs. 100% of peds
neurology is all off label, it is immoral to do a double blind study
on a small child so most drugs say not for use in children under the
age "_".  None of the drugs used on emily have been efficiently in
babies but they try different amounts until they get the right
result.  I would read all of the side effects and make your choice
from there

I do not feel my comment in any way was attacking and/or biting your head off, I was merely pointing out to lana when she asked that she should go by her child's needs and what not.   emily should get 800 some odd calories by the chart but her nutritionist says 1200 and we only get 750 in a day and she does not gain well but does gain some.   Like if your child is supposed to get 1000 calories per day but only takes in 800 but seems fine you should use that as your guide.  After reading a lot of replies I did not feel as though any were argumenative, just additive to the original post, though I have not read them all

I hope you (or anyone else) don't find my comments in any way attacking.  Often I feel I sound cold in email.  Much is lost in email as a communication process.  I read people's emails and often cry because of their struggles and yet what I write lacks often the care that I felt when I wrote it. Communication is hard.  What is intended to be communicated is rarely exactly what is received.

I just don't want anyone to get carried away and think I'm saying that it's the only way to go and no one else is right b/c every child is different (as many have pointed out) and has different need's. I just kind of assumed (my mistake) that everyone knew their child was different and depending on their need's/health issues may require something other.
This was just a figure and chart given to me by a child nutritionist and very similar and agreed by our pediatrician. Also... (one more thing) all ped's/nutritionists are different as well depending on your area, their experience with preemies, etc.......

you are going to find this with a lot of drugs. 100% of peds
neurology is all off label, it is immoral to do a double blind study
on a small child so most drugs say not for use in children under the
age "_".  None of the drugs used on emily have been efficiently in
babies but they try different amounts until they get the right
result.  I would read all of the side effects and make your choice
from there

I would greatly appreciate it! I did do a lil research on it this morning and as I had thought it's used for psychiatric/behavioral issues and according to the info has been commonly used in children for more than 10 yr's.... however there wasn't an age range though so that wasn't very helpful. I guess I could give you the dosage amount and you could mention that along with age just to see what opinion they have to offer. It's a very small dosage... She said something totally opposite, she said that there is very minimal swelling which is common in children and that her ears looked great and the tests were all fine showing no signs of anything related to the ears, throat, tonsils, adenoids, etc.......  How frustrating!!! So as a last resort the only thing I could think of was maybe something going on brain wise that we don't see an d called her neurologists who wasn't far away from the ENT doc. I had to carry on and on to get in b/c it's almost impossible to see him at times but she finally called me back on my cell before leaving town (which is 2 hrs one way from our house) saying that she had a 3:45 cancellation so we ran around a bit then went over there. The doc checked her out, measured her head, asked lot's of question's, and I explained the degree of it and how long it's been going on as well as her sleeping as lil as 3-4 hrs in a 24 hour period at times b/c of it. After checking her out he said he believe's she is experiencing a compulsive obsessive behavioral issue b/c of her brain delay's and using it as a way to express herself and get out her frustration's since she is delayed. So... he want's to try her on a behavioral medicine before bedtime each night to see if it help's... I guess it can't hurt.  I pray this is the solution and it help's and I get my happy lil girl back. I asked him how sur e of this he was and he said he's almost certain but to be on the safe side he would order a MRI but since she's uncooperative she has to be fully sedated and we can't get in until the 15th. Then we see him on the 21rst for a follow up. Anyway's, that was our long and very frustrating day... oh yes, and that was all after not being to bed since 1:30 this morning when my dd decided to wake up screaming with a tantrum after only 2 hours of sleep.

I do not feel my comment in any way was attacking and/or biting your head off, I was merely pointing out to lana when she asked that she should go by her child's needs and what not.   emily should get 800 some odd calories by the chart but her nutritionist says 1200 and we only get 750 in a day and she does not gain well but does gain some.   Like if your child is supposed to get 1000 calories per day but only takes in 800 but seems fine you should use that as your guide.  After reading a lot of replies I did not feel as though any were argumenative, just additive to the original post, though I have not read them all

Aya,
  Let me point out that this chart is just based on weight/size and has nothing to do with activity levels and was also given to me for babies/toddlers and not older children. I just don't want anyone to get carried away and think I'm saying that it's the only way to go and no one else is right b/c every child is different (as many have pointed out) and has different need's. I just kind of assumed (my mistake) that everyone knew their child was different and depending on their need's/health issues may require something other.
This was just a figure and chart given to me by a child nutritionist and very similar and agreed by our pediatrician. Also... (one more thing) all ped's/nutritionists are different as well depending on your area, their experience with preemies, etc.......

Risperdal... have you had experience with it before? I have somewhat with my nephew who I helped raise but I never really did much research on it. When I worked in nursing (nearly 4 yr's ago now) I only worked with the elderly so didn't see it much there either but I think it's more of a drug you see used with behavioral or ADHD kid's. I plan on researching it some today now that I have had a lil rest.

 About a week ago I posted an update on my dd's compulsive violent ear pulling and that the pediatrician said he believed it was b/c her tonsils were swollen irritating her adenoids which was giving a pressure feeling in the ear and she would more than likely need a tonsilectomy. Well after nearly a month of no sleep, pulling ears, and screaming I cancelled her therapies today to take her to the ENT specialists b/c the med's the ped put her on have been useless to say the least. After a forever wait the doctor looked at her and decided to do some test. Here's the kicker (grr!!!) She said something totally opposite, she said that there is very minimal swelling which is common in children and that her ears looked great and the tests were all fine showing no signs of anything related to the ears, throat, tonsils, adenoids, etc.......  How frustrating!!! So as a last resort the only thing I could think of was maybe something going on brain wise that we don't see an d called her neurologists who wasn't far away from the ENT doc. I had to carry on and on to get in b/c it's almost impossible to see him at times but she finally called me back on my cell before leaving town (which is 2 hrs one way from our house) saying that she had a 3:45 cancellation so we ran around a bit then went over there. The doc checked her out, measured her head, asked lot's of question's, and I explained the degree of it and how long it's been going on as well as her sleeping as lil as 3-4 hrs in a 24 hour period at times b/c of it. After checking her out he said he believe's she is experiencing a compulsive obsessive behavioral issue b/c of her brain delay's and using it as a way to express herself and get out her frustration's since she is delayed. So... he want's to try her on a behavioral medicine before bedtime each night to see if it help's... I guess it can't hurt.  I pray this is the solution and it help's and I get my happy lil girl back. I asked him how sur e of this he was and he said he's almost certain but to be on the safe side he would order a MRI but since she's uncooperative she has to be fully sedated and we can't get in until the 15th. Then we see him on the 21rst for a follow up. Anyway's, that was our long and very frustrating day... oh yes, and that was all after not being to bed since 1:30 this morning when my dd decided to wake up screaming with a tantrum after only 2 hours of sleep.

Yeah, my child isn't even on that many calories and he is a bit fat weight to height ratio says so).   Its a hard balance.  We want him to grow in height to catch up, so we push the calories as far as we dare, but we aren't even giving as many as on this chart for his weight.  We are doing 860.  He just weighed 21 yesterday at the Pediatrician.   He seems plenty active too.   -- Edith

  This is probably true depending on the child. I used the chart b/c it came from a friend of mine who has been a child nutritionist for around 10 yr's now and has a 3 yr old 24 wk'r of her own... so I figured who better. I also have got a weight based figure from the ped which is very similar as well, but you are right... every child is different so each parent just has to use their better judgement when it comes to their child. I was just giving her what she asked for........ *smile*

This is not meant to be critical, and I'm sure this chart is accurate
for most "regular" kids!!!  Phillip's dietician said you also need to
take into account the activity level of the child.  If, for some
reason, your child is more or less active than other babies, his/her
caloric needs will be different.  My son is an extreme case...very
hypotonic, but his dietician found he gained the "right" amount of
weight with about 650 calories (at 18.5 pounds).

Well after nearly a month of no sleep, pulling ears, and screaming I cancelled her therapies today to take her to the ENT specialists b/c the med's the ped put her on have been useless to say the least. After a forever wait the doctor looked at her and decided to do some test. Here's the kicker (grr!!!) She said something totally opposite, she said that there is very minimal swelling which is common in children and that her ears looked great and the tests were all fine showing no signs of anything related to the ears, throat, tonsils, adenoids, etc.......  How frustrating!!! So as a last resort the only thing I could think of was maybe something going on brain wise that we don't see and called her neurologists who wasn't far away from the ENT doc. I had to carry on and on to get in b/c it's almost impossible to see him at times but she finally called me back on my cell before leaving town (which is 2 hrs one way from our house) saying that she had a 3:45 cancellation so we ran around a bit then went over there. The doc checked her out, measured her head, asked lot's of question's, and I explained the degree of it and how long it's been going on as well as her sleeping as lil as 3-4 hrs in a 24 hour period at times b/c of it. After checking her out he said he believe's she is experiencing a compulsive obsessive behavioral issue b/c of her brain delay's and using it as a way to express herself and get out her frustration's since she is delayed. So... he want's to try her on a behavioral medicine before bedtime each night to see if it help's... I guess it can't hurt.  I pray this is the solution and it help's and I get my happy lil girl back. I asked him how sure of this he was and he said he's almost certain but to be on the safe side he would order a MRI but since she's uncooperative she has to be fully sedated and we can't get in until the 15th. Then we see him on the 21rst for a follow up. Anyway's, that was our long and very frustrating day... oh yes, and that was all after not being to bed since 1:30 this morning when my dd decided to wake up screaming with a tantrum after only 2 hours of sleep.
