Hi -

I'm new to this group.  I have a 2 year old with almost total
unwillingness to eat.  He has a g tube because of another medical
diagnosis, and we are looking for a feeding clinic.  I've researched
them on the net, and there don't appear to be any in our area, so if
we attend one, we expect to travel.

We are expecting to go to one that treats daily for several weeks.
Does anyone have any experience with this type of clinics?  Any
recommendations, or "DON'T" recommendations?

I'm guessing overall cals. but too bad she is hungry.  And I
wouldn't be a good mother letting her be hungry.

She is doing better with the change now.  I have upped her fruits
and veggies even added oatmeal to all meals to help slow up her
tummy just a bit.

I plan on trying cream of wheat next.

I can't really tell if she has gained much weight but she seems to
be a bit more tired after school now.  But who knows if it is the
food or just being a hard working 1st. grader.

Hi everyone. I am new to the group and would like to share my little
ones feeding problems. My daughters name is Ashley and is a 25weeker,
twin girl, weighing only 1 lb 10oz at birth. She has been battling
with feeding problems since her discharge from the NICU at about 3
1/2 months old. She was diagnosed with GERD at 4 months old showing
all the classic signs, refusing to eat her bottle and arching her
back with feedings. She was tried on many different medications and a
couple different formulas. She continued to gain wt until march 04,
10 months old. At that time we made a tough decision and she had a
nissen and g tube placed. The most important thing, she starting
gaining wt again but many problems came along. After the sx she
started gagging and wretching but bringing nothing up. About a month
later she started bringing a little bit of mucous up. Now, 6 months
after the fundo she constantly coughs, gags, and vomits tons of
mucous. She is now 100% tube fed and refuses to take anything by
mouth. It is so disappointing and aggrevating trying to figure out
what is going on. She is on continuous feedings at night and every
time she wakes up, in the middle of the night or in the morning, she
starts coughing which leads to her gagging and then vomiting mucous.
After every bolus during the day she gags and brings up mucous. She
has seen many different dr.s including a couple gi dr.s,
pulmonolgist, allergist, neurologist, and ENT. She is on neocate
formula. Her meds include prevacid, zantac, and emycin. She recently
had an endoscopy done which showed loosening of the fundo and GERD
with esophogitis and gastritis. She has feeding therapy 3 times a
week. She is very willing to try to eat but when she does it triggers
her to gag and bring up mucous. I am just wondering if her GI issues
are causing some pulmonary issues that is causing all this mucous
production. Given her hx, she was a 25 weeker on the vent for about 1
1/2 months. We have an appt with the GI dr. and pulmonologist this
week so I am hoping they can answer some of my questions. The
therapists feel she will not eat by mouth until her medical issues
are resolved. She doesn't feel safe to eat. I am not sure if there
are any pulmonary meds that can help with her problem. Sorry so long
and that you for your help.

We never knew about the flavor packets until we were off of the
Neocate. So we ended up using Splenda to sweeten the formula
(recommended by the GI). It worked. It still tasted pretty nasty,
but it helps.

We've taken your advice and switched him over to the Neocate.  The
only problem is now - he hates the taset so much that he won't eat
it.  I have noticed less mucus in his vomit so that is a releif -
but how do you get your child to take the neocate?  Any flavoring
ideas?

the ear infections have probably  been one thing that  keeps her from eating better.
 
who did you find out about her milk allergy?   was she tested for any other allergens? 
 
by the way if you want more info on  feeding a kid with allergies join
 
list for parents of food allergic kids.  very helpful if you feel like you are running out of things to offer her.   they can find milk free version of just about anything.  like creamier smoother soups for her to try. they have experience with using WIC with allergies too.
 
a feeding therapist is usually a speech therapist or occupational therapist who  has further specialized.
 
does she seem to have sensory issues in general?  does she like messy play for example.
 
I hear  eating in general is harder for folks with impaired vision.

Thank you so very much for the "Steps to Eating"!  Those are very helpful!
 
since she's already getting some therapy ask if they have a feeding therapist too.
I didn't realize there was such as thing as a feeding therapist.  Her OT gave me some tips of things to do, but they aren't helping.  They are mostly sensory things to desensitize her face.
 
any way you can  flavor her formula?  maybe if it were a bit different she'd try it gain.
As for the formula, I tried putting Carnation Instant Breakfast into it, so it would be chocolate.  She still refused it.
 
I'd put safe margarine into the potatoes and maybe even  some pureed veggies, like baby food.
We put extra margarine into her potatoes.  As for the pureed veggies, she can tell by the smell and then refuses them.  If you then try to give her some regular potatoes, she will refuse those also and then won't eat anything.
 
margarine on the crackers too?
She doesn't like the feel of the moist margarine with the crunchy crackers.  She spits them back out and then will refuse to even eat plain crackers.
 
 will she dip these foods in a sauce?
She hates anything dipped.  She won't even eat ketchup on fries.
 
is she old enough to try soy milk?  not as nutritious as formula but does have some  vitamins added and has calories
I'm not really sure to be honest, but she gets WIC and they won't give us soy milk.  I, however, am waiting for her nutritionist to call me back to see what we can do.
 
I assume you've had her rechecked for the ear infection to make sure it's really gone.   are ear infections common for her?
She was diagnosed with an ear infection on October 4th and we went back on October 25th to see if it was gone.  Her ear was a little red, but was not infected.  I had to take her back to the ped on October 27th because guess what, her ear was infected again.  Yes, ear infections are extremely common for her.  One more and she will be getting tubes.
 
maybe you can figure out where she is with various foods and get an idea of the next baby steps to take
Thank you so much for these.  Fruits and vegetables she tolerates on her plate and that's it.  She won't do anything with them.  Some foods, she will bite, chew "x" times and spit out.  The "Steps to Eating" is very helpful!  Thank you so much!

We use this with our son who is night tube fed.  It works great.  If
he does wrap up in it, since it is stiff, it doesn't wrap tight (we
also run the tube down the end of his pajamas so it will wrap is
legs and not his head). He also loves to play with it.  Our recent
problem is that he has figured out how to take it off.  So, we just
put a little tape on either end of the cable loom and a piece in the
middle so he can't remove it (fortunately he hasn't figured out how
to remove the tape yet).  We found ours at Wal-mart for less than $3
(at Wal-mart I think it is called flexi-tube and if I remember right
it was in the automotive section).

Good luck.  Wrapping himself up in the tube is always a big worry of
mine.

Many families have found that running the feeding tube through corrugated split plastic tubing works very well.  It is available in 1/2 inch and 1 inch diameters and lengths up to 20 feet.  This is used primarily to organize cables from home theaters, computers etc. for protection and neatness.  They also can be used with feeding tubes, either for continuous daytime feedings or nighttime feedings.   One of the companies that has a very wide selection of lengths, diameters and colors is:  Cable Organizer.  Their product is called Wire Loom.  You can see a description, pictures and pricing at:   http://cableorganizer.com/wire-loom/colored.html

The cable tubing  allows you to slide the feeding tube in at the vertical split in the tube.  It is stiff but also quite flexible.  It moves easily but protects the cables/tubes.  It also makes interesting sounds when the child moves his fingers along the corrugated tube, so this is often more satisfying to children than trying to pull on the tube.

Thank you for the information!  Kristy's throat was looked at by the ped when she was diagnosed with an ear infection.  She said everything looked fine.  I don't use other milks with Kristy.  As far as the high calorie juice, she can't drink much juice because she only has one kidney.  What is Duocal?  As for soup, no she won't eat it.  She hates the combination of the liquid and noodles or vegetables or whatever.  Again, thank you for all the information!

do the baby food stops   match any other gains he's made?  often a kid will backslide in one area when they're  working on another.  like grasping or crawling or walking.

Here is a question some one might be able to answer. My son is 100% G-tube
dependent and is also non-verbal and I have had tons of people ask me if he
"could feel it going in?"  For those of you that have verbal gtubers - do
they feel it?

 
my dd can certainly feel when she's getting full but I don't think she feels it like stream coming into the stomach.  I don't think the stomach nerves are that precise about feeling movement of food within the stomach. otherwise we'd all feel it when our stomach are digesting I'd think.

wow that is a really high concentration, when emily could not take in enough calories to sustain we were on a PICC of partial TPN and lipids
that lasted about 15 months or so but it has its own risks

This happened to my already low-appetite son (no other health or physical problems, though once diagnosed with silent reflux), when he got his first ear infections at @ 11-12 months.  The illness and the antibiotics put him off of ALL creamy, sticky and gooey foods (before that, he would eat yogurt, cream cheese, etc.).  He still won't take these things.  Whenever he is sick (he will be two on Friday), he selects down to just a few things--french fries, pretzels, crackers and hot dogs were his mainstays too  (though he has added a number of other things as his staples over the past year--apples, eggs, cheese, bread, etc.) .  He does start to add more foods when he gets better, but is still very picky (and a low-quantity eater).  He has gone through phases of the chewing and spitting out, and I've found that it coincides with a throat ailment or teething.  You should have your daughter's throat checked--she might have some residual sore throat. It is all very frustrating, but I've gotten used to it and try to remind myself that he will pick back up a bit again when he is totally better.  I know you can't exactly do this with your daughter, but during these times, I really boost his milk drinks (usually yogurt-smoothie milkshakes) so that he gets SOMETHING.  Do you use other milks with your daughter?  I know that there is a high-calorie juice out there.  Boost as much as you can.  I use Duocal in my son's shakes everyday.  Will she eat soup?

Kristy was eating a variety of foods; however, about four weeks ago,
she got an ear infection.  Since then, the foods that she will eat
have dwindled greatly.  She now eats crackers, french fries,

eats 2 or 3 of one thing and starts chewing up the food and spitting
it back out.  She refuses to swallow it.  She also has a milk allergy
and is on formula; however, as of recently, she has decided not to
drink formula.  She was born with one kidney; therefore, she can't
have much juice, so the things she can drink are limited. 

She currently gets Occupational Therapy, Vision Therapy as she also
has vision processing problems, has myopia, and strabismus  She also
has limited lower vision.  And she also gets Physical Therapy. 

My current concerns are that she only eats a select number of foods
and in very small quantities.  Also, that she mostly chews up food,
but only to spit it back out.  She also is very limited as to what
she can drink and is refusing her formula.

Hi Mike -

Papaya enzyme helps with the digestion of protein. I tried it on my
son without much success (evidently digesting protein isn't what is
causing his motility issues) but I know other people have said it
worked for them. The brand we used was "Papayazyme" made by "Arise
and Shine" and I believe you can purchase it directly through their
web site.

I don't know of a "natural" appetite stimulant but if you find one,
please post. I would like to have Andrew try it. Periactin is a
prescription appetite stimulant and several people on this site use
it. For faster digestion, erythromycin is commonly used. It's an
antibiotic but one of the side effects is that it causes the stomach
to contract stronger so it's frequently used in very small doses for
motility. (Andrew takes .5ML 4x/day.) Reglan is another motility
drug, but it can have negative side effects so I believe more doctors
are turning to e-mycin. If you use e-mycin, you may want to try to
feed Brandon baby yogurt or use a probiotic to replace the "good"
bacteria the antibiotic will kill.

A few other thoughts... does Brandon have a weak suck? If so, have
you tried a Haberman feeder? It's designed for infants with
difficulties sucking. His swallow study may have been normal, but
they may not have looked at the actual suck. Also, a swallow study
is "a moment in time" and may not be a true indicator of what is
actually happening.

Another thought, it sounds as if Brandon may be eating just enough to
become satiated (sp?). Does he act like he's in pain when he's eating
or if he becomes overly full? If so, his lack of desire to eat may be
more than behavioral. These little guys commonly have GI issues. Have
you seen a pediatric GI doctor? Does your feeding therapist think
it's entirely behavioral?

You mentioned in your previous post that you were thinking of
changing peds. I am just now learning how important it is to trust
your gut instinct. If you feel there's a problem, chances are there
is. Just because your doctor isn't concerned doesn't mean everything
is hunky dory. We saw one ped GI who instantly decided my son needed
a repeat fundo (long story). I was not pleased with the diagnosis and
sought a 2nd opinion. Our new ped GI not only thinks Andrew does NOT
need a repeat fundo, he also believes his first surgery may have been
the root cause of many of his GI issues. It makes me sick to think
that my son's problems eating may be compounded by the fact that I
trusted blindly every doctor in the NICU instead of researching and
exploring my options.

You might want to look into papayazyme to increase stomach motility.

the NICU approx. 50 days. One of my sons, Brandon, while deveolping
normally in everything except his weight and length. He has had
feeding problems (not eating enough) ever since he came home from the

Usually, he has 4oz breastmilk mixed with 1oz of heavy whipping cream
and sometimes 2tbsp of rice cereal. This is to add calories to his
diet. Out of this he will usually take only about 2 - 2.5 oz before
he doesn't want anymore. He takes this very slow about 30min - 1hr .
He eats/drinks extremely poorly in the morning where he only takes
about 1oz at 6am and just a little better at 10am.  He does much
better at his 6pm feed.

We started adding the heavy whipping cream about 3 weeks ago to add
extra calories to his diet. He did extremely well the first 2 weeks
but this last week he has dropped from drinking 28 - 30oz per day to
18-20oz per day. He has refused any baby food where he was starting
to progress in this area too.  He does seem to be putting on more
weight now but it is such a struggle to feed him.

He was taking babyfood at around 6-7 months.  He could eat about 1/2
a jar at a feeding but then he stopped altogether after about a month
to a month and a half..  He starting eating babyfood again after we
starting going to his feeding therapy but now he totally refuses to
eat again (after about 2 weeks), ok may two or three bites
(unwillingly).  I think that

I would like to find something to increse his appetite as well as
help him empty his stomach more quickly.

One of our doctors told us not to increase the calories past 32.  I
don't remember why though.
