I am not discounting the discussion but this is a forum for nurses to
discuss emergency nursing issues. There are plenty other fora out there to
discuss
immunization concerns

I agree. It is this type of ill-informed, emotional response that leads to

CDC is just as engaged in trying to determine the cause of autism. There is
not a single study that can directly link vaccinations with autism. At best
it links the age of vaccinations to the time autism emerges. My young
cousin has autism. It has been difficult for the family and we know the
path ahead will be difficult for Adam.
I am bothered with such knee-jerk reactions as those below because it
attacks an organization of people who have dedicated their lives to save

fyi -- hmmmm, Paul McCarthy's wife:

03/07/2008
JENNY McCARTHY: JULIE GERBERDING MUST GO!
By Jenny McCarthy

I'm asking all parents and autism groups to join me in demanding
Julie Gerberding's immediate resignation as Director of the CDC.

On Monday, March 10th, beginning at 9:00am Eastern Daylight Time,
let's all start calling the White House and ask President Bush &
Laura Bush to demand Julie Gerberding's resignation for
incompetence during the autism epidemic. The White House switchboard
can be reached at:

202-456-1414

Also, on the same day, please call your local Congressperson and
Senators from your state and ask them to call for her resignation,
too.

Julie Gerberding has led the CDC for 6 years during a time when the
autism epidemic has only gotten worse. Despite tens of thousands of
children who declined just like Hannah Poling, Ms. Gerberding stood
before cameras yesterday defiant, cold, and defensive. Where is her
humanity in the face of such tragedy? Why couldn't she have
said, "We at CDC want to make sure what happened to Hannah doesn't
happen to any other children, we want to make vaccines safe"?

Rather than listen to the heartbreaking stories of so many parents,
you can be sure that Ms. Gerberding is spending her time right now
trying to get the Spin Machine up and running to minimize, confuse,
and deceive the American public.

The autism epidemic won't end until we fix the vaccine schedule by
reducing total vaccines, separating shots, waiting until our kids
are older to begin shots, greening our vaccines, and screening for
at-risk kids. Ms Gerberding has stood by and watched self-interested
parties more than triple our vaccine schedule and I'm certain her
inactivity to help our kids will continue.

The chances of Ms. Gerberding taking the radical steps to reform the
CDC and reform our vaccine schedule to make it kid-safe are zero! We
need a new CDC Director who is an open-minded reformer and who
recognizes that we are experiencing an epidemic of autism, which Ms.
Gerberding has never publicly admitted.

Please, parents and national autism organizations, let's all help
make our voices heard on Monday.

Thank you,

Jenny McCarthy

This does not provide enough information regarding your circumstances.

This is my opinion:

1st always listen to the family. If they exceed your time limits tell them, "I
have 2 minutes to listen." Set boundaries.

   2nd give choices.

3rd contact a patient rep. In my job, I have access 24/7 to a patient rep to
handle pt. complaints.

4th safety first. Watch for family/patients escalating. Take a SAFE class if
you have the opportunity. Call security and explain the circumstance to them.

5th. Validate the patient/ family concerns. "I understand what you are saying
is... and I can see how that is very concering to you. Let me put you in touch
w/ the dr. who can then talk to you more."

6th If this is a dr. related issue, I don't get in the middle. I directly refer
the source to the doctor.

7th offer choices. Give avenues of options.

8th if they are very aggressive find out your policy about having security
escort them out to the lobby or out of the hospital. Chances are if they have
anger issues, others are targets too in their life.

I hope that these ideas help. Workplace violence is very concerning but never
put yourself in harms way.

I have to recommend the FRontline online self taught training programme for
working with difficult situations. Originally intended for working with severe
personality disorder it has now become a self taught learning tool growing
in popularity. It is free to register and you learn at your own pace using
very real scenarios. I think it is excellent and has been set up in the UK by
the Department of health, St Georges medical School adnd others .

See _http://www.frontline-training.org/_ (http://www.frontline-training.org/)

Anyone can register and do the on line modules.

Reassessments are every 2 hours in the department and in the triage area. In
triage, it is 2 hours for level 3 and lower (we use the ESI 5 level system).
Level 2 and higher get vitals and reassessment related to their complaint
and stability but never less than 2 hours. Level 4 and 5 hopefully aren’t
with us that long!

How often do you reassess your patients in the ED? I understand it depends
on the acuity. Does anyone use 5 level triage? If so, do you tie those
levels into your reassessment in the ED?

Phillip,

I too recommend that every nurse gain experience in an emergency room! My
thoughts are that nursing intution and the fundamentals are best learned on a
med surg unit for at least 1-2 years, in my opinion.

some units want you to have some med/surg experience before coming into the

it makes a difference. You will come to the ED with some confidences with your
skills and a wider knowledge base. One good thing about nursing is the door is
open to just about any speciality you would like!

If you have the chance, go work in the ED as an extern, if you have not already,
just to get a taste of what it is like!

Good luck in school and in your future as a nurse!!
