Dear Sir/Mam,



I am Benzon Castillones Bsn Rn,A member of DX5LYM 5,otherwise known
as ACCESS 5.We are a non stock non profit organization composed of young
radio amateur and Emergency Rescue enthusiast in Tacloban City ,Leyte
Philippines. Currently we are attached to and the emergency support
service of the Regional Disaster Coordinating Council of Tacloban City.
Our purpose of this organization and aim is to help and support the
people of Tacloban City in terms of communication thru Ham radio and the
delivery of emergency support services in times of disasters and
calamities.

Currently we are in the process of developing a Medical Assistance
Group. In this regard we are asking donations from any individual or
charitable organizations with unused medical equipments such as
Ambulances, oxygen tanks, Medical Logistics, stretchers, backboards,
spine boards, Ems bags, Ambu Bags and Etc, broken vehicles in which we
will fix later. We will shoulder all the transportation and shipment
fees.

Our only aim is for the service of the people of Tacloban City and
the entire Leyte Province. In which this area is a typhoon prone and
earthquake belt areas.
