The extremity should be immobilized at the level of the heart not
higher. The circumference of the affected extremity should be
measured and re-measured every 15 to 60 minutes or so to monitor
progression of the edema. Remove any rings or constricting items from
the involved extremity. Apply a compression dressing over the bitten
area with an elastic wrap. The wrap should be tight enough (but with
fingers still able to slide under the dressing) to reduce lymphatic
flow. Compartment syndrome is rare but can occur. O2 by nasal
cannula or mask, 2 large bore IV's cardiac monitoring and pulse
oximetry, fluid boluses of normal saline or lactated ringers if
patient is hypotensive, and get them to the ER. Hope this helps.
