I work in a teaching and educational hospital in Houston, TX. We use the 5 tier
system as well. Levels 1, 2, and 3 are easy to judge. But 4 and 5 are
questionable. Sometimes you are not sure if they will need only xray or meds
also. It is different with each doctor. It is sometimes difficult to access
the resources that will be needed beforehand. We have been using this system
for about 6 mon now.

Chicago, IL USA. We use the 5 tier system. The one
problem I have with it is the difference between 3, 4
and 5. Perhaps a patient will only need an Xray and
is therefore a 4, but that doesn't mean he shouldn't
go before a 3 who will need another resource or two,
especially if the 4 is in a lot of pain.
