It is true that in the US the docs hold all the cards but ... thank heaven ... itīs not quite the same here ... in Sweden the hospitals are government run and non profit ... midwifes have free hands ( almost ) in delivery and as far as I know docs are called in only occasionally..... personally I had 2 difficult LONG deliveries with depressed foetal heart function in the second one ( they kept trying to "wake" the baby ... )I was closely monitored but was delivered by my midwife of a daughter with cord once about her neck ( apar 9, 10 ,10 ) ...
the only time the doc got involved was when I had my epidurals ( sigh ... but when you are heading into the second day of full labour with only 2 cm progress all those birth plans go out the window )
this is the norm ... unless they need a vacuum extraction ( is that what it is called ... when they pull ( ahem ...help ... ) the baby out with a suction cup ? ) or a c-section
homebirth is in itīs infant stage ( pun intended ) but growing here .......mainly I believe because we have had midwife managed births as a norm for so long that women have been less pressed to search for an alternative .... they are now government subsidised and therefore FREE for the woman ... same as hospital births .... they are cheaper for the system and therefore I believe they will soon increase in numbers
I would love to be a home birth midwife but feel that I would need a lot of births under my belt first
I loved the ward otherwise ... sweet babies and milky moms ... tears and smiles and the rapture of the first meeting... the time i spent there this time was right in the middle of rounds ... I hope things will be different next time ( tomorrow)

In a hospital enviroment CNM's are as you say a Physician Assistant.
But many go into private practice as a collaborative partner or as a
sole provider with a physician back-up doing home births. It is all
about power and money. Physicians do not want to lose either as well
as the hospital which gives out it's addimitting privledges/attending
designations, so the hospital pulls in the progressive Doc who will
give you a back-up physician and the hospital keeps it's clients and
thier money. L&D is one of the few wards that has a profit!
So now many with that insight you may want to look at a birthing
center for a job when you graduate or pursue the Certified
Professional Midwife designation rather than the CNM so that you may
focus on homebirths. Here in Ohio as I understand it CNM's can NOT
attend homebirths, whereas a CPM is not recognized by the state as a
midwife (only CNM's) so they are not confined and can attend
homebirths. The problem is finding a friendly Doc to back you up.
Again it boils down to the control the hospital has it and the
profits that go with it and put up a fight everytime the issue of
homebirths or having the state recognize the CPM designation.
Do not loose faith and talk about what you see and how it all impacts
you (feelings) this will help keep yours and my dreams alive as well
as the other members here and those who may join in the future! It is
these type of messages and helping each other that will provide
comfort and incentives/motivation during these changing times.
