The CT scan is fairly recent, third one in the past 10 years.  I also have sleep apnea but was unable to use a CPAP. We tried two different machines and four or five face pieces but they all felt like that scene in "Alien" where the creature attaches to a guy's face.
 
Maybe I'll take Dr. Tichenor up on his offer. What's one more doctor?
 
That is good news about your son; nothing is worse than a child with a potentially major problem. 
 
Speaking of pain, I recently had another tooth infection.  While waiting for the antibiotics to work, I was popping Vicodin like they were M&M's.  I could kill the tooth pain but not completely eradicate the headache.  I don't think my doc believes that completely.  Oh well. He's not the first.

It does sound possibly sinus related, doesnt it, which is why I was surprised  to hear nothing turned up on the CT.  Was this done recently?  Perhaps it would be worthwhile having another done, in case something was missed first time around.
Another thought I had, although you probably have looked into it already, is sleep apnea.  My husband suffers terrible headaches related to sleep apnea, always present on waking or shortly after-- now he is on a CPAP machine at nights it has made a big difference  for him.
I  joined this group in the hopes of learning more about a rare  neurological condition my 4 yo son has, called CPH- chronic paroxysmal hemicrania.  It is controlled by daily doses of a NSAID,  Indomethacin.  It took a year of testing  looking for brain tumor, benign intercranial hypertension, and various other conditions before a diagnosis was made.  Thankfully the indomethacin has helped enormously, although he does suffer with extended periods of break through pain, normally relieved by increasing the med dosage. 
 
I would seriously consider taking up Dr Tichenors' offer --I cant imagine being in such pain for such a long time as you have been!  He may just be the dr that sees what others have  missed!

He said he would be happy to see me if I wanted to go to New York but really couldn't offer much encouragement.  I'm still thinking about going.  My wife and I are over due for a nice vacation...

Hi, How did the indocin work for you? I take varapamil(for high blood pressure too) and celexa(anxiety), but they also help migraine. Regards, longell...
