I think this is a great idea - however, will there be a way to determine which
answer is the most accurate with a final stamp of approval on the answer. I am
sure there may be several ways to code a particular example and there may be
more than 1 correct answer but having a definitive answer is always a good
thing. Thank you all.

Rule H6 in the Multiple Primary & Histology manual would apply to this
case: Code 8523/2 (intraductal carcinoma mixed with other types of in
situ carcinoma) when there is a combination of intraductal carcinoma
and one or more specific intraductal types

[Cancer Registrar Group] Question 1
Here's the first question/case.
Pathology report reads: Right breast mass excision, high grade ductal
carcinoma in situ, solid and cribriform types, w/cancerization of
lobules and associated calcifications. No invasive carcinoma.
(Comment: DCIS presenting very focally and spreads only several
millimeters in size.)
What would be the histology code?

Here's the first question/case.
Pathology report reads: Right breast mass excision, high grade ductal
carcinoma in situ, solid and cribriform types, w/cancerization of
lobules and associated calcifications. No invasive carcinoma.
(Comment: DCIS presenting very focally and spreads only several
millimeters in size.)
What would be the histology code?
As the founder of this group I hope to start posting coding/staging
questions on a regular (every couple days) basis so that we can
all "chime" in and give our own ideas on how the situation should be
coded/staged. Maybe we can start using this as a learning tool
since the group is not getting much use lately.
For any of you out there that has a question they'd like asked,
simply post it. Make sure to be as detailed as possible so that you
will recieve the right answers. It doesn't matter how simple the
question seems or how difficult. If you know the answer but it is
an interesting or unusual case, post a question to see how others
would handle it.
If you post a response, be sure to put the question in your response
so that we will know which question you are referring to....if there
are multiple questions floating around out there. Also, please try
to refer to the source from which you found the answer.
If you are not set up to receive notices from the group by email,
you may want to change it so that you can be included in the
questions and answers. You can set it up where you will received
one email a day with all the posts from that day so that you will
not be inundated with emails - assuming we have a lot of response
from this!!!
Thank you so much for sharing.

It was pretty short sited not to have anticipated the interest in this
presentation.
I have heard that the July web conference will have unlimited logins to
both see and hear the presentation.
However, if it consists of reading the slides and manual...of well, I'll
still log on in July to get the exposure I need to do 2007 cases.
See you at the webconference...!

I heard that there were over 300 registrars logged on, so the great
majority did not have audio. A registrar that I know said that a lot
of people did not mute their phones so you could hear a lot of
conversations going on.
Also the audio portion consisted of reading what ever was on the
slide and the pertinent page from FORDS, with no further explanation
or enhancement.
Although I appreciate the effort at education, frankly I agree with
Linda. My time would have been better utilized just reading the
information for myself.

This is not reportable unless the pt. had a bx. in the hospital or had

Subject: [Cancer Registrar Group] Reportabilibty Question
Reportability: If a patient has a prostate bx in the physician
office, comes to our facility with a week or two for a staging work up
that doesn't show mets, then goes elsewhere to have seed implants, is
he considered reportable for us and if so, what class of case? As far
as I know, he has disease because he has not had any treatment yet.
What if I do not know he is going elsewhere for treatment? If the
physician is a staff physician, and it is unknown if he had tx then
would he be COC 1? And, what about staging? Will these require
staging?

Subject: [Cancer Registrar Group] FTE Survey
I looked at the NCRA website and printed off the Work force study but
it is almost 7 years old now and in registry terms that is
almost "obsolete"!! !
So, I was thinking.... why don't we do our own survey? I know of some
online survey sites that we can use for free though they do have
limitations. ...like the number of questions and/or the number of
participants.
I know many years ago we did a salary survey on this group but it kind
of fell short due to lack of man power to complete the study. Using
an online survey site we will get the results tabulated on the site
and not have to do any stats ourselves.
If you all would like to do this, email me or post here and we'll work
on it. Also, give me suggestions on questions. They should be
multiple choice type questions.
My email is lorilindsey@ charter.net or Loraine.Lindsey@ piedmont. org.

I've heard there is a study being discussed in regards to what hospitals
are doing just state reporting or have a full blown registry doing
studies, follow-up, complete abstracting. All of this will be taken into
consideration. For example in 1981 there were 41 data items per abstract
and in 2006 we have today over 200 data items. There were 10 or less codes
to choose from for any one data item in 1981 and there are now are 50 or
more codes for each data item. Also in 1981 there were 4 areas of the
cancer program surveyed and now there are 9 areas surveyed. There were no
specific standards in 1981 and we now have 32 standards. I did not compile
this but someone else did and this person now works for the COC.
