How should I record the PSA level in collaborative stage in the
following situation?

The physician states the patient had been followed for years with an
elevated PSA. The patient was put on Proscar. The PSA continued to
be elevated at 7.38 on Proscar. The actual number is 3.69, but the
conversion factor was 7.38 because the patient was on Proscar. The
patient underwent TRUS that revealed adenoca, Gleason 3+3, 6.
