I'm posting the information that I received from CAP regarding
specific protocols. I'll list them each separately by cancer site
for ease of reference.
For The Hodgkin lymphoma questions, as you can see below, you have
the option of specifying which specific lymph node site was biopsied
or state site not specified. And as for the tumor size, you can
either give a dimension if you know it is only one node and it was
all taken out you can specify a size, otherwise you can state cannot
be determined and write in a comment why. As for node regions
involved and other organ involvement, you can state it if you have
pathologic tissue involvement, otherwise you can say not specified.

Comment: Although the neoplasm measures 4.5 cm in the biopsied
lymph node, it uncertain whether other lymph nodes are involved and
a definitive size cannot be determined.
