Pathologists at our facility complete the staging form before the attending
physician receives it. As registrar I send all staging forms to the
pathologist and then on to the attending physician--attending physician
being the one that performs the most definitive procedure/care. After
completion by physician the staging form become part of the permanent
medical record.

So when do you send the staging form to the pathologist, 4-6 months after diagnosis when you are abstracting? ... attending ... physician ... After ... ...

Our cancer committee has set a time limit on when the staging form has to be sent. The form has to be to the pathologist within 30-45 days of procedure or dx...
