Pathologists at our facility complete the staging form before the attending
physician receives it. As registrar I send all staging forms to the
pathologist and then on to the attending physician--attending physician
being the one that performs the most definitive procedure/care. After
completion by physician the staging form become part of the permanent
medical record.
