Thanks for your help. The other cancer registrar check the AJCC 6th
edition and found documentation that malignant pleural effusion does
in fact make it a stage IV. Since the beginning of the section does
not list this as being a change from the 5th edition, we will assume
that it means stage IV in the 5th too.
