BASIC TRAINING IN CANCER DATA COLLECTION AND REPORTING

Buffalo, NY – October 28-November 1, 2003

PRE-CERTIFICATION REVIEW COURSES

New York City – February 5-7, 2003
Albany – February 10-12, 2003
Rochester (Victor) – February 17-19, 2003

ALL REGISTRANTS MUST:

Be employed by a health care facility or live in the State of New
York

Complete the enclosed registration form or register on-line at
cppsinc.com on the "Classes and Trainings" page, at least two weeks
prior to the start of the course

Bring required reference books

Wear proper business attire to class

For Pre-Cert Review classes, registration must be accompanied by a
RECENT letter (on hospital letterhead) from your supervisor stating
that you are eligible to sit for the CTR exam. (NOTE: Admission to
class will not be permitted without this letter)

If you register on-line, you must bring your eligibility letter with
you to class.

These classes have been updated to include use of ICD-O-3, General
Summary Staging 2000 and new reportable terminology.

There is no charge for this course or handouts, but travel, lodging
and all meals are your responsibility.

Class size is limited to 35. Space allocated on first-come, first-
serve basis.
