I am now using the oral motor protocol to assessment and motor function and
sensory responses. And I also use a form to invite parent to write down what the
child like (accept/like/crave) and dislike (dislike/ refuse) in vegetables,
meat, fruit, snacks, and others.

I also use Sensory Profile and Sensory Integration clinical observation test to
assess overall sensory processing function towards different stimulation.

Could anyone give me some sugguests about what else could be used for assessing
picky eater? Is there a scale for assessing oral or orofacial hypersensitivity ?

For intervention, I am now using Wilbarger protocol and gum massage (with/
without food in pureer format). Systematic approch in increasing food texture
and food taste varities. Texture, Taste, Amount, change just 1 variables at a
time.

What else intervention strategies could be used ? Could anyone share their
approaches in handling picky eater ?

I'm trained in both of those pgms as well. Also, Food Chaining.

It always takes a while after a GI bug to regain foods. Just think how u feel after one. If it takes us -adults- a week, then it will take a picky eater at least 2wks, if not more.

I would enroll the client back in therapy target more variety of foods, esp bland ones.

Bland foods for dehydration--applesauce, popsicles. I would need to know much more about what foods already eaten, age, etc. before making further rec.

QUERY
Hi Debra,
I remember that during the conference in Seattle that several of us had asked you for the "Steps to Feeding Protocol" or guide you had. This has come to be of great importance. I have a co-worker who has had some training in another feeding program. She wants to  use another feeding protocol to co-treat with this child. Would you please e-mail me this protocol.
 
I also wondered if you had made a CD or DVD with a step by step how to perform your exercises.
 
RESPONSE
 Attached is the information on the Beckman Sensory Motor Protocol, three documents.  I make certain that the person has 50% at least for midblade elevation, posterior cheek strength and jaw strength before focusing on food.  I ask the family to work outside of mealtime for new foods and use familiar foods during the meal.  I work on increasing quantity of familiar foods first, then increase variety at the lowest texture level tolerated to increase to 20 different foods, then increase texture last.  By the time we are ready to work on increased texture, I find that the individual has usually already increased texture on most foods on our list, and in therapy, we only have to work on a few areas, usually vegetables and meats.
I do not have a video or DVD for sale.  I am working on adding a shopping cart feature to the web site which would enable me to have pay per view with pass word log in for therapists who have already taken the training.  I am also working on an online CEU for several topics, including this one.  Would you be interested in field testing it when it is ready?

The following is a query received off list-serve with response.

Hi Debra-

A colleague and I were discussing how to score lip and cheek strength
and we came across a question for you.  First of all, we work with
children-usually between the ages of 2 and 4.  When you are testing
for either of these and are giving verbal cues for the child to
activate that muscle by saying something or having them "get your
finger", how do you account for if you did not feel the contraction
because they did not understand the direction?  We know that this
contraction should be felt if the person is not cognitive or
conscious on a reflexive basis, but does this also apply to the young
child?  Can you feel the contraction in a child who is awake even if
you do not ask them to respond as listed above?

DEBRA BECKMAN'S RESPONSE
I am so glad you are completing baselines for your caseload.
The child does not have to be able understand commands, the response
then becomes reflexive.  However, if the child does understand
commands, and you think they are not responding just to vex you, I
would score it the way you feel it - 0/5 for cheek strength or 0/6
for the lip strength.  You can always update the baseline and I would
rather underestimate than over estimate.  Some 2 and 3 years olds can
and do follow commands.  Many understand the command, but have
specific oral motor weakness at these small levels of activation that
make it impossible for them to activate, just like the older lady on
the case study we showed in class.  Muscle range and strength are not
cognitive, they are mechanical properties of the muscle.  The
response is active if the person is cognitive, and reflexive if they
are not cognitive.  It is possible to be cognitive and have poor
range or strength, which would result in failure for the muscle to
activate for these small tasks on the Beckman Oral Motor Protocol.

The following question was received off list. The question and
response in posted to the group for further discussion or questions.

Question:
I currently have a 1.4 year old patient who is in desprete need of
these exercises; however, she has a seizure disorder and is going to
start medication soon.  Is it okay for me to do an assessment and
intervention for this child, or will the oral motor stimulation cause
seizures?

Response:
Movement of the muscles of the face does not set off seizure
activity.  Seizures may occur that are unrelated to what ever you are
doing for interventions on the face.  Make certain the head is always
supported to avoid any injury should a seizure occur during therapy.
Also avoid working in a room with UV lights that may flicker and set
off seizures.

Debra,

It has been almost a year since we met up in Kentucky about my "oral
stim" study with premature infants.  I wanted to let you know that I
just finished data analysis on the study, and although it was just a
pilot (n=19), it showed very promising results.  The preterm infants
who got the "Beckman Oral Motor Intervention-Premature Infant"
version (remember how we adjusted the original protocol from 15
minutes down to 5 minutes..and so I am calling that the Premature
Infant version) got through the transition from tube feedings to
bottle feedings 5 days sooner than the control group.  AMAZING!!!!
We only did the intervention once a day for 7 days.   So it looks to
be very publishable, and I'll be writing the manuscript early this
fall.

I also wanted to ask you again if it was still OK for me to write up
a short manuscript on the revisions of your original 15 minute
protocol to the 5 minute version for preterm infants.  And if so,
would you be interested in co-authoring that with me?

thank you for replying. After serious reflection over this thesis project, I have come to the same conclusion as you, and have changed my focus to a more holistic centered approach. I am not including any specific hands on exercises in my recommendations to parents, but am focusing on the person-environment and parent-child relationship interactions, which has such a big impact on and is so important to the feeding and mealtime experience.  I am citing your publications and seminars in the lit review of the thesis, however, and would appreciate your correct citations.

I do not give permission for publication of the Beckman Oral Motor Interventions without the direct input of the therapist treating the child.  I have had several incidents of misuse of the information to the detriment of the child with negative impact for all of the therapists using Beckman Oral Motor Interventions.  You may certainly reference the material, but you may not publish the actual protocol and interventions for general use.  Do you have the correct citation information?

Hi Bonnie,

What were the findings on the Beckman Oral Motor Protocol.  To increase chewing on food, the child must have more than just jaw strength.  To eat textured foods, the child needs posterior cheek strength, the tongue movement that you mentioned as well as the ability to chew 20 times at one per second on left and right.  When these skills are in place, I then use the Beckman Sensory Motor Approach and work outside mealtime, as we discussed at the conference.  I am attaching these files to this email.  I recommend increasing quantity of favored foods to 4 ounces in 20 minutes, then work on variety at the texture that is the easiest for the child until there are  20 different foods that the person can eat (5 each of protein, starch, vegetables and fruits).  Then work on increasing texture for those foods.  I work on only one food at a time that the family chooses based on the above information and we all work on the same food outside of mealtime.  The mealtime is a family and social time using whatever food for intake is the easiest for the person to eat.

the other.

Teaching cortically blind kids is a slow process. We have been working
on chewing for 6+ years. Are there any ideas out there??

I do not give permission for publication of the Beckman Oral Motor Interventions without the direct input of the therapist treating the child.  I have had several incidents of misuse of the information to the detriment of the child with negative impact for all of the therapists using Beckman Oral Motor Interventions.  You may certainly reference the material, but you may not publish the actual protocol and interventions for general use.  Do you have the correct citation information?

officially started on March 24.  Our son began to use the straw
consistently on April 20.  We totally think this was accomplished
thanks to the Beckman method.  He is drinking a variety of liquids
like water, sodas, juices.  We live in Puerto Rico, but we are
planning to get a re-evaluation every time we go back to Orlando.

I'm looking for ideas to work with a cortically blind low CD child age
11 years.  She can bring a loaded spoon to her mouth and remove the
food but does not chew.  A recent refresher of the Bechman course
helped set up a program for tongue movements and chewing activities.
However, she still isn't doing any active chewing. Just one bite then
swallow.

There is great school staff involvement with daily carryover of
activities.  A gauze food bolus was used a couple years ago but given
up on.  Food is chopped fine to prevent choking. The flat spoon is used
to present food into her cheek/molar area with little to no chewing
noted.

Currently doing pressure to the middle of her tongue and sides of her
tongue as well as circular swipes within her mouth.  A probe is placed
into her molars with 4-6 very slight chews on one side and one bite on
the other.

Teaching cortically blind kids is a slow process.  We have been working
on chewing for 6+ years.  Are there any ideas out there??

I am trying to track the effectiveness of the therapy techniques. I
already take before and after pictures but wanted to add some numbers
to the data so I could show results and protect client
confidentiality.  I want to track frequency provided, duration of
service, time of day provided.  I was going to do the Beckman
assessment at set intervals to measure progress.  Eventually, I want to
be able to prove that the exercises can be done in a school setting and
the same excellent results can be obtained.  This is my first time
trying to collect data of this type so any suggestions would be
appreciated.

Welcome to the inaugural members of the Beckman Oral Motor Research Institute list serve!  This site is a part of the vision of the future for therapists who are using Beckman Oral Motor Assessment and Intervention Protocol..  It is my hope that through on going collaboration nation wide and world wide, each of us as clinicians can assist in building the body of research needed to guide the application of this method for those individuals we treat who are not able to move the muscles of the face as needed for important and basic functions such as control of secretions, eating, drinking, facial expression and speech.  Your input is critical to this endeavor.  Much work remains to be done and we are the ones to do it.

We may try to look at sensation seeking behavior in several ways: e.g. He may seek it for joyful experience, which I would interpret it as seeking the things that he need or he want. He may also seek it for cancellation, i.e. some sensation make him uncomfortable, which will make him even more uncomfortable when those senses come unexpectedly, so that he may try to get strong input from other senses(which is he can expected, because he seek it by himself) to "cover" those unwanted sensation. 
 
SI assessment could help to figure out the sensory processing function of the child. And is the child having "sensory diet" ? If so, you may have to know the content of the "diet" and communicate with the parent on the effect of it, so that you may consider use some of those activities in your treatment session together with the oral motor training. See if you can liaise with the OT and communicate with her to work it out together. Because in my practice, for those kids with SI and Oral issue, I tend to do oral motor training together with sensory diet to improve the effect of both.
