Conference Overview - Increasingly, care is provided from a wide range of professionals, often geographically remote from each other.  It is essential that each has access to the very best and up to date information about the patient for whom they provide. The conference seeks to examine and review initiatives to 'join up' the provision of care across traditional boundaries and to look forward into how technology might provide resolutions to some of the problems information availability in support of shared care.
